Abstract: Introdução: A empatia é fundamental na relação médico-doente, havendo evidência sólida da sua associação a melhores resultados na prática clínica. Contudo, a investigação atual sugere que há declínio da empatia durante o Mestrado Integrado em Medicina (MIM). A avaliação da evolução da empatia nos estudantes ao longo do MIM da Faculdade de Medicina da Universidade de Coimbra (FMUC) é útil para identificar lacunas na formação médica e melhorá-la.Objetivos: Avaliar a evolução dos níveis de empatia nos estudantes do MIM da FMUC, descrever diferenças entre sexos e aferir se o sexo é determinante na evolução da empatia.Metodologia: Estudo observacional longitudinal realizado através da aplicação da Escala Jefferson da Empatia Médica – versão para estudantes (JSPE – spv) aos estudantes do MIM da FMUC, em dois momentos distintos do percurso académico. O questionário foi aplicado a alunos do 1º, 3º e 4º ano de MIM (ano letivo 2015/2016) e foi novamente aplicado quando estes já se encontravam no 3º, 5º e 6º ano de MIM (ano letivo 2017/2018) respetivamente. Foi realizada análise estatística descritiva e inferencial, admitindo-se um nível de significância de 5%.Resultados: A amostra total incluiu 750 estudantes de medicina da FMUC, 232 do ano letivo 2015/2016 e 518 do ano letivo 2017/2018. Na evolução do 1º para o 3º ano verificou-se aumento da pontuação global da JSPE total (p=0,002) e do componente “Tomada de perspetiva” (p<0,001), mas diminuição do componente “Compaixão” (p=0,003). Neste grupo existiu efeito isolado do sexo com pontuação superior no sexo feminino para a “Tomada de perspetiva” (p<0,001) e no sexo masculino para a “Compaixão” (p<0,001). Do 3º para o 5º ano observou-se aumento da pontuação da “Tomada de perspetiva” (p<0,001) e diminuição da “Compaixão” (p<0,001). Ainda neste grupo, encontrou-se pontuação superior no sexo feminino para a “Tomada de perspetiva” (p=0,003) e no sexo masculino para a “Compaixão”(p<0,001). No estudo da evolução do 4º para o 6º ano constatou-se diminuição global da JSPE total (p=0,023) e o sexo masculino apresentou pontuação superior na JSPE total (p=0,026) e na “Compaixão” (p<0,001). Quando analisada a interação entre ano de MIM e sexo, verificou-se que o sexo masculino influenciou positivamente a progressão da pontuação no componente “Tomada de perspetiva” (p=0,025) do 1º para o 3º ano, e negativamente no componente “Compaixão” do 1º para o 3º ano (p=0,002) e do 3º para o 5º ano (p=0,015).Conclusão: Os níveis de empatia oscilam ao longo do MIM na FMUC, salientando-se uma diminuição acentuada, sem influência da variável sexo, nos últimos três anos (predominantemente clínicos). Os resultados encontrados ilustram a necessidade de identificar e integrar medidas que potenciem o comportamento empático no plano de estudos da FMUC.
Introduction: Empathy is essential in the doctor-patient relationship, there is a solid evidence of that association to better results in clinical practice. However, current research suggests that exist a decline in the empathy during the Integrated Master’s degree in Medicine (MIM). The evaluation of the evolution of empathy in the students along the MIM of the Faculty of Medicine of the University of Coimbra (FMUC) is useful to identify gaps in medical education and to improve it.Objectives: To evaluate the evolution of empathy levels in FMUC students of MIM, to describe differences between the sexes and to determine if sex is determinant in the evolution of empathy.Methodology: Longitudinal observational study carried out using the Jefferson Scale of Physician Empathy – students portuguese version (JSPE – spv) to FMUC's MIM students, in two different moments of the academic course. The questionnaire was applied to students in the 1st, 3rd and 4th year of MIM (academic year 2015/2016) and was reapplied when they were already in the 3rd, 5th and 6th year of MIM (academic year 2017/2018) respectively. A descriptive and inferential statistical analysis was performed, assuming a level of significance of 5%.Results: The total sample included 750 medical students from FMUC, 232 from the academic year 2015/2016 and 518 from the academic year 2017/2018. In the evolution from the 1st to the 3rd year, there was a global increase in the total JSPE (p=0.002) and in the "Perspective taking" component (p<0.001), but it presents a decrease in the "Compassionate care" component (p=0.003). In this group there was isolated effect of sex with higher score in females for the "Perspective taking" (p<0.001) and in male to "Compassionate care" (p<0.001). From the 3rd to the 5th year, there was an increase in the score of the "Perspective taking"(p<0.001) and "Compassionate care" (p<0.001). Also in this group, a higher score was found in females for "Perspective taking" (p=0.003) and in the male sex for "Compassionate care"(p<0.001). In the evolution study from the 4th to the 6th year, there was a global decrease in the total JSPE (p=0.023) and the male presented a higher score in the total JSPE (p=0.026) and in the "Compassionate care" (p<0.001). Analysing the interaction between MIM year and sex, it was verified that the masculine gender influenced positively the progression of the score in the component "Perspective taking" (p=0.025) from the 1st to the 3rd year, and negatively the component "Compassionate care" from the 1st to the 3rd year (p=0.002) and from the 3rd to the 5th year (p=0.015).Conclusion: Empathy levels fluctuate over the duration of the MIM in FMUC, with a marked decrease, with no influence of the sex variable, in the last three years (predominantly clinical). The results present the need to identify and integrate measures that enhance empathic behaviour in the FMUC study plan.
