Abstract: Introdução: O tabagismo é uma das principais causas evitáveis de morte prematura e os dados recentes corroboram a perspetiva de que está associado a inúmeros tipos de cancro. Não havendo um estudo da prevalência e incidência de neoplasias associadas ao tabagismo no nosso país, analisar a prevalência de hábitos tabágicos (por sexo e faixa etária) em doentes diagnosticados com neoplasia, através do estudo dos registos clínicos, torna-se pertinente, pois permite concluir quais os valores reais da frequência desta patologia na população da área geográfica de atuação da Administração Regional de Saúde do Centro, com o objetivo de alertar a comunidade médica e científica para eventuais lacunas existentes nestes e de sensibilizar as populações para a problemática do tabagismo. Métodos: Após consentimento ético, analisamos as bases de dados cedidas pela Administração Regional de Saúde do Centro, organizadas por sexo e faixa etária. Usamos como filtros os códigos ICPC-2 (International Classification for Primary Care) relativos a neoplasias de diversos aparelhos (sangue e linfáticos, digestivo, respiratório e urinário), para estudar os indivíduos com diagnóstico de neoplasia, e o código ICPC-2 P17 (abuso de tabaco), para estudar a prevalência de hábitos tabágicos neste grupo e na população total. Realizamos a análise estatística com recurso à plataforma R, package fmsb, e também ao software IBM SPSS v24, utilizando o teste exato de Fischer com um nível de significância de 0.05.Resultados: Na população total, encontramos uma prevalência de hábitos tabágicos de 9% e, nos indivíduos com diagnóstico de neoplasia, verificamos uma prevalência de 7,6%, sendo que a maior associação se verificou para neoplasias dos aparelhos respiratório [odds-ratio 1,99 (p<0,001)] e urinário [odds-ratio 1.04 (p=0.278)] e a menor para neoplasias do sangue e linfáticos e do aparelho digestivo, apesar destas corresponderem às neoplasias mais frequentes na população. Encontramos maior associação de hábitos tabágicos nos indivíduos do sexo masculino com idade igual ou superior a 65 anos, assim como, neles, maior frequência de neoplasias. Discussão: Os resultados obtidos da análise dos registos clínicos de pessoas com a classificação ICPC2 para neoplasia estão de acordo com as últimas publicações. Estes apontam para a existência de uma associação significativa entre neoplasias e a prevalência de hábitos tabágicos, revelando diferenças entre as frequências do grupo de fumadores e do grupo de não fumadores. Os resultados que representam a prevalência de P17 na população total foram inferiores ao relatado em prévios artigos, sendo necessário estudar como foram realizados os registos, para que seja possível confirmar a sua veracidade. Conclusão: A incidência crescente de neoplasias pode ser justificada por fatores comportamentais e estilos de vida, e neste estudo o tabaco demonstrou estar significativamente associado. Considerando os benefícios da cessação tabágica e a importância da prevenção primordial para a saúde das populações, os estudos epidemiológicos são uma ferramenta fundamental para a obtenção de conhecimento sobre estas doenças e os seus fatores de risco, sendo essencial garantir que existem bases de dados fiáveis e que possam ser utilizadas no futuro pela comunidade médica e científica.
Introduction: Smoking is one of the main causes of premature death and recent data corroborate it’s association with several types of cancer. In Portugal there is a lack of studies in the prevalence and incidence of smoking-associated neoplasms and, as such, it is important to study clinical records, in order to analyze the prevalence of smoking habits (by sex and age group) in patients diagnosed with cancer. By studying records from patients living in the geographical area of the Health Administration of Portugal's Central Region, we aimed to alert medical and scientific community to possible gaps in its medical records and raise public awareness for the problems smoking can be associated with. Furthermore, we investigated the frequency of smoking habits registered.Methods: After ethical consent, data bases results provided by Regional Health Administration of Central Region (ARS Centro) were analyzed, organized by gender and age group. The ICPC-2 codes (International Classification for Primary Care) were used as filters, relating to cancer of the blood and lymphatics, digestive system, respiratory and urinary tracts, to study the individuals with diagnosis of cancer, as well as the ICPC-2 P17 code (tobacco abuse), to study the prevalence of smoking habits in this group of individuals and total population. The R platform was used for statistical analysis, in particular the fmsb package, as well as statistical software IBM SPSS v24. Fischer's exact test was used with a significance level of 0.05. Results: 9% of the total population has smoking habits. Of all registered patients diagnosed with cancer, 7.6% are smokers, the greatest association was found for respiratory tract cancer [odds-ratio 1, 99 (p<0,001)] and urinary tract [odds-ratio 1.04 (p=0.278)] and the lowest being cancer of the blood and lymphatics, even though these are the most frequent neoplasms among the population. We found a greater association of smoking habits among males 65 years of age and older, as well as a higher frequency of cancer. Discussion: These results based on the analysis of the clinical records of patients with ICPC2 classification for cancer are consistent with the latest publications. They point to the existence of a significant association between neoplasms and the prevalence of smoking habits, revealing differences in the frequencies between the smokers group and the non-smokers group. Prevalence of smoking habits (P17) in the total population was lower than the one reported in previous articles, and it is pertinent to study how the records were registered, in order to confirm their veracity. Conclusion: The increasing incidence of cancer can be justified by behavioral factors and lifestyles, and this study demonstrates that tobacco and cancer are significantly associated. Taking into account the benefits of smoking cessation and the importance of primary prevention for the populations health, epidemiological studies are a fundamental tool for obtaining knowledge about this type of disease and its risk factors, thus, it is essential to guaranty reliable data bases that may be used by medical and scientific communities in the future.
