Abstract: O objetivo deste artigo de revisão visa estudar a clínica, os métodos de diagnóstico e o tratamento da infeção crónica por Vírus da Hepatite E em doentes imunossuprimidos. Também visa fazer um apanhado geral da realidade portuguesa em relação ao vírus, tanto a nível epidemiológico como a nível clínico.De acordo com os estudos revistos neste artigo, a infeção por Vírus da Hepatite E é relativamente comum, mas, no entanto, muitas vezes acaba por ser subdiagnosticada devido à sua clínica inespecífica. Na população imunocompetente a infeção é breve e resolve-se sem necessidade de terapêutica. Em doentes imunossuprimidos esta infeção pode tornar-se crónica, quando causada pelos genótipos 3 e 4 do Vírus da Hepatite E. Os estudos referidos neste trabalho indicam também que, na maioria dos casos, a infeção crónica é assintomática, sendo o sintoma mais comum a fadiga. Analiticamente a alteração mais comum é a elevação das transaminases hepáticas. Quando surge uma infeção crónica esta vai induzir o aparecimento de fibrose hepática e, consequentemente, pode ocorrer descompensação hepática e morte. A infeção crónica pode estar associada a manifestações extra-hepáticas, as mais importantes das quais surgem a nível neurológico, mas também podem ocorrer a nível renal, hematológico e outros. O diagnóstico de infeção crónica faz-se por pesquisa de ácido ribonucleico do vírus, e considera-se crónica quando está presente há mais de 3-6 meses.Conclui-se também que, atualmente não existe uma terapia com indicação para o Vírus da Hepatite E. Nos casos de infeção aguda, dado que esta se resolve espontaneamente, não é realizada terapia, exceto em doentes imunossuprimidos, ou em alguns casos específicos. Nas infeções crónicas em doentes imunossuprimidos, a terapia consiste inicialmente em reduzir a intensidade da imunossupressão, quando esta está a ser realizada. Caso esta estratégia não seja bem-sucedida inicia-se terapia com ribavirina durante 3 meses.No caso concreto de Portugal verifica-se que não existem estratégias de prevenção nem rastreio para a infeção por Vírus da Hepatite E para a população imunossuprimida. Alguns países europeus já adotaram medidas de prevenção através da pesquisa de vírus em doações de sangue. Na China desenvolveu-se uma vacina para o Vírus da Hepatite E, no entanto, esta é dirigida para o genótipo 1 do vírus, que é pouco prevalente em Portugal.
The goal of this review is to study the clinic, diagnostic methods and treatment of chronic infection by Hepatitis E Virus in immunosuppressed patients. It also aims to show the Portuguese reality about the virus, such as epidemiology and medical knowledge. According to studies reviewed in this article, the infection by Hepatitis E Virus is common, however, it is often underdiagnosed due to its unspecific clinic. In immunocompetent population it is characterized by a brief infection which resolves without the need of therapeutics. In immunosuppressed patients the infection can become chronic when it is caused by the genotypes 3 and 4 of the Hepatitis E Virus. Studies cited in this work show that in most cases the chronic infection is asymptomatic, being fatigue the most common symptom. Laboratory analysis most frequent change is an elevation of the hepatic transaminases. When there is a chronic infection this will induce hepatic fibroses and, consequently, there can be an hepatic decompensation and death. The chronic infection can be associated to extra-hepatic manifestations, being neurologic the most important ones. There can be also renal, hematologic and other extra-hepatic manifestations. The diagnosis of chronic infection is done by searching ribonucleic acid of the virus, and it is defined as chronic when it lasts for at least 3-6 months.It was also concluded that there is no therapy with indication for the Hepatitis E Virus. In the cases of acute infection there is no need for therapy since it resolves spontaneously, except in immunosuppressed patients, or in some specific cases. In chronic infection in immunosuppressed patients, the initial therapy is lowering the immunosuppressive therapy. In case of failure of this strategy it is indicated to start therapy with ribavirin for at least 3 months.In Portugal there are no prevention or screening strategies for the infection by Hepatitis E Virus in the immunosuppressed population. Some European countries have started prevention measures with the screening of blood components. In China, there is a vaccine against the genotype 1 of the virus, which has a low prevalence in Portugal.
