dc.description.abstractIntrodução: O carcinoma do recto tem uma elevada incidência, representando a segunda causa de morte por cancro na Europa. O prognóstico dos doentes depende de variados fatores, nomeadamente do estádio TNM, da invasão venosa extramural, linfovascular e perineural bem como da resposta à terapêutica. Até à data não foi identificado qualquer marcador tumoral quantificável no soro capaz de predizer o outcome dos doentes de forma independente. Objetivo: Este trabalho tem por objetivo fazer uma revisão dos estudos publicados nos últimos 10 anos que relacionam os níveis séricos de CEA baseline, pós-quimioradioterapia neoadjuvante e pós-cirurgia com o prognóstico dos doentes com carcinoma do recto localmente avançado. Métodos: Realizou-se uma pesquisa sistemática em várias bases de dados, com inclusão dos trabalhos que abrangiam os doentes com carcinoma do recto em estádio II e III e submetidos a cirurgia precedida ou não de quimioradioterapia neoadjuvante. Estes estudos tinham como variáveis em estudo os níveis séricos de CEA pré e/ou pós-terapêuticos e os seguintes outcomes: sobrevivência global, sobrevivência livre de doença, resposta patológica completa e risco de recidiva local. Resultados: Foram selecionados 53 artigos elegíveis, sendo que 74,0% dos resultados obtidos estabeleceram a correlação do CEA com os outcomes definidos. Observou-se uma maior percentagem de estudos a confirmar a relação do CEA com a sobrevivência global (77,3%) e livre de doença (80,0%) em comparação com a resposta patológica completa (68,8%) e o risco de recidiva local (60,0%). 73,9% dos estudos que incluíam o CEA pós-quimioradioterapia neoadjuvante apontavam para o seu valor como fator de prognóstico no carcinoma do recto. Conclusões: O CEA parece ser um importante fator prognóstico e preditivo de resposta à terapêutica no carcinoma do recto, destacando-se o CEA pós-quimioradioterapia. No futuro, será pertinente o tratamento estatístico dos dados contidos neste trabalho sob a forma de meta-análise.por
dc.description.abstractIntroduction: Rectal cancer has a high indicence, representing the second cause of death by cancer in Europe. The prognosis of patients depends on a variety of factors, especially the TNM stage, lymphovascular, perineural and extramural venous invasion, as well as the response to therapy. To date, there is no mensurable tumor marker identified that is capable of predict the outcome of patients as an independent factor. Aim: To review the studies published in the last 10 years that relate the baseline, post-neoadjuvant chemoradiotherapy and post-surgery CEA levels with the prognosis of patients with locally advanced rectal cancer. Methods: A systematic search was carried out in several databases. Studies that comprehended patients diagnosed with rectal cancer stage II and III and submitted to surgery, whether or not preceded by neoadjuvant chemoradiotherapy, were included. The variables analyzed in these studies were the pre- and post-therapeutic CEA serum levels and the following outcomes: global survival, disease-free survival, pathologic complete response and risk of local recurrence. Results: 53 eligible articles were selected and 74.0% of the results confirmed the correlation of the CEA with the defined outcomes. It was observed a higher percentage of studies confirming the relationship of CEA with overall survival (77.3%) and disease-free survival (80.0%), compared to the pathologic complete response (68.8%) and the risk of local recurrence (60.0%). 73.9% of the studies that included the post-neoadjuvant chemoradiotherapy CEA levels indicated its value as a prognostic factor in rectal cancer. Conclusion: CEA seems to be an important prognostic and predictive factor of response to therapy in rectal cancer, standig out the post-neoadjuvant chemoradioterapy CEA levels. In the future, it will be pertinent to perform a statistical analysis of the data contained in this work in the form of a meta-analysis.eng
