Resumo: Introduction and Objectives: SNS 24 integrates the National Health Service (SNS) and offers a telephonic line assured by a team of nurses, which is available 24 hours per day, 365 days per year and plays an important role in the provision of health care. The usage of this platform has increased significantly throughout the last couple of years and it is very important to evaluate its impact in health in Portugal. The objective of this study was to analyse the suitability of the hospital referral by this service.Material and Methods: A descriptive, retrospective and unicentric observational study was performed in the first quarter of 2018, through an analysis of the clinical records of the patients observed in the Emergency Department (SU) of Hospital Pediátrico de Coimbra (HPC), referenced after contact with the SNS 24. Sociodemographic variables, telephonic call contextualization, the adequacy of the medical referral and the concordance between the motive of referral and the final diagnosis were analysed.Results: 1.051 cases were included. The average of ages was 2.0 years (7 days - 17 years old) and 54.8% were males. The telephonic contact occurs mainly on Sundays between 4 p.m. and 12 p.m. Cough was the symptom that motivated the most referrals (27.0%) and the indication was for observation at the SU in almost all cases. It was attributed urgent or very urgent priority to 53.9% of the patients. Acute nasopharyngitis was the most common diagnosis (18.2%). One third of the patients were submitted to complementary diagnostic tests. In more than half of the cases (58.0%) only symptomatic measures were recommended and 95.1% of the patients were discharged.Discussion and Conclusion: Having in mind the assigned triage at the SU of HPC, more than half of the cases were properly managed. Analysing the little or nothing urgent cases, a group (11.8%) that required complementary diagnostic tests or therapy at the SU was identified, justifying the referral. The absence of primary health care on weekends and at night can be one of the factors to contribute to the referral of these cases to the hospital emergency. The referral was considered unjustified in 40.7% of the patients. If on the one hand the telephonic triage can overestimate the gravity of the cases, on the other hand, the parents’ health illiteracy can contribute to increasing the perceived severity of the disease. It is concluded that although the majority of the hospital referral is appropriate, there are many cases without severity forwarded to the hospital emergency, in which a better access to primary health care and an investment in parents’ education could have an impact.
Introdução e Objetivos: O SNS 24 integra o Serviço Nacional de Saúde e dispõe de uma linha telefónica assegurada por uma equipa de enfermeiros, disponível 24 horas por dia, 365 dias por ano, que presta cuidados de saúde. A utilização desta plataforma tem aumentado ao longo dos anos, sendo importante avaliar o seu impacto na saúde em Portugal. O objetivo deste estudo foi analisar a adequação do encaminhamento hospitalar por este serviço.Material e Métodos: Tratou-se de um estudo observacional descritivo, retrospetivo e unicêntrico, no primeiro trimestre de 2018, através da análise dos registos clínicos dos doentes observados no Serviço de Urgência do Hospital Pediátrico de Coimbra (SU-HPC), referenciados após contacto com o SNS 24. Foram analisadas variáveis sociodemográficas, a contextualização da chamada telefónica, a adequação do encaminhamento hospitalar e a concordância entre o motivo de referenciação e o diagnóstico definitivo.Resultados: Foram incluídos 1.051 casos. A mediana de idades foi de 2,0 anos (7 dias - 17 anos) e 54,8% eram do género masculino. O contacto telefónico ocorreu mais frequentemente ao domingo e entre as 16.00h e as 24.00h. A tosse foi o sintoma que mais motivou referenciação (27,0%) e a indicação foi para observação no SU na quase totalidade dos casos. Foi atribuída prioridade urgente ou muito urgente a 53,9% dos doentes. A nasofaringite aguda foi o diagnóstico mais comum (18,2%). Um terço dos doentes realizou exames complementares de diagnóstico (ECD). Em mais de metade dos casos (58,0%) foram recomendadas apenas medidas sintomáticas e 95,1% dos doentes tiveram alta.Discussão e Conclusão: Tendo em conta a triagem atribuída no SU-HPC, considerou-se encaminhamento adequado em mais de metade dos casos. Analisando os pouco ou nada urgentes, identificou-se um grupo (11,8%) que necessitou de ECD ou de terapêutica no SU, podendo justificar o encaminhamento. A ausência de cuidados de saúde primários (CSP) ao fim de semana e à noite pode ser um dos fatores a contribuir para a referenciação destes casos à urgência hospitalar. O encaminhamento foi considerado injustificado em 40,7% dos doentes. Se, por um lado, a triagem telefónica pode sobrestimar a gravidade dos quadros, por outro, a iliteracia dos pais para a saúde pode contribuir para aumentar a noção de gravidade da doença. Conclui-se que, apesar de uma referenciação hospitalar maioritariamente apropriada, há ainda muitos casos sem gravidade encaminhados para a urgência hospitalar, nos quais um melhor acesso aos CSP e um investimento na educação dos pais poderia ter impacto.
