Title: Intervenção do farmacêutico comunitário na melhoria da qualidade de vida do doente asmático

Abstract: Introdução: A asma é uma patologia crónica que tem um grande impacto na qualidade de vida dos doentes. Deste modo, cabe ao profissional de saúde dar o máximo de informação sobre a doença, promover a adesão à terapêutica e assim melhorar a qualidade de vida destes doentes. Objetivos: O presente estudo tem como objetivo avaliar o grau de conhecimento do doente asmático sobre a patologia, a adesão à terapêutica, o controlo da asma e rinite alérgica e o modo de utilização dos dispositivos inalatórios. Com os resultados pretende-se consciencializar os profissionais de saúde e as entidades sobre a importância de implementar programas educacionais para os doentes asmáticos. Metodologia: O estudo foi realizado na Farmácia Moço, em Coimbra, tendo sido convidados a participar todos os utentes maiores de idade com diagnóstico de asma (reportado pelo doente) e com terapêutica instituída. O presente trabalho integra um estudo mais alargado que envolveu quatro farmácias comunitárias do distrito de Coimbra (3 na cidade de Coimbra e 1 na Figueira da Foz) entre abril e junho de 2013. O objetivo deste trabalho foi, por um lado recrutar novos asmáticos para o estudo, e por outro reavaliar alguns doentes avaliados no decorrer do ano passado. O estudo consistiu na realização, pelo entrevistador, dos seguintes questionários: Questionário de Conhecimentos Sobre a Asma (QCSA), Questionário de Morisky-Green, Teste de Controlo da Asma e Rinite Alérgica (CARAT) e Teste de Controlo da Asma (ACT). Após o preenchimento dos questionários, efetuou-se a espirometria recorrendo ao Vitalograph COPD-6, com determinação dos parâmetros FEV1, FEV6 e FEV1/FEV6 pré-broncodilatação e a observação da técnica inalatória. Resultados: Nos nove novos doentes, o número de respostas corretas ao QCSA variou entre 13 e 22, apenas um adere completamente ao tratamento, três apresentam um bom resultado ao CARAT, cinco não revelam qualquer problema no seu estado de saúde, quatro avaliaram o seu controlo da asma nas últimas 4 semanas como bem controla e cinco mais ou menos controlada segundo o ACT. No momento da entrevista, todos os doentes apresentavam valores de FEV1 superior a 80% e três doentes não executaram corretamente a técnica inalatória. Nos 2 doentes reavaliados, um doente continuou a executar a técnica inalatória incorretamente, apresentou melhor resultado no FEV1, melhor classificação no QCSA, mas pior classificação nos restantes questionários. O outro doente continua a executar 6 corretamente a técnica inalatória e a esquecer-se de tomar a medicação. Apresenta resultado superior no QCSA mas inferior nos restantes questionários. Conclusão: Os dados obtidos neste estudo revelam informações importantes sobre as necessidades dos doentes asmáticos. Os resultados mostram que é necessário a intervenção do farmacêutico comunitário para aumentar o grau de conhecimento dos doentes sobre a doença, aumentar a adesão à terapêutica, explicar a técnica inalatória, permitindo assim controlar a patologia e melhorar a qualidade de vida destes doentes.
Introduction: Asthma is a chronic illness which strongly affects the patients’ quality of life. So, it is up to the health experts to give the maximum amount of information about the illness and promote the use of therapeutics so as to better the patients’ quality of life. Objectives: The objective of this study is to evaluate the asthma patients’ degree of knowledge about the illness, the use of therapeutics, controlling the asthma and allergic rhinitis, the patients’ quality of life and how to use inhaler devices correctly. The findings in this study are intended to bring awareness to health experts and corporations to the importance of implementing educational programmes for asthmatic patients. Methodology: This study was conducted at “Moço” chemists in Coimbra. The customers who were invited to participate were all over 18 years of age and reported themselves as asthmatic and following treatment for asthma. This paper is part of a broader study which involved four community chemists from the district of Coimbra (three in the city of Coimbra and one in Figueira da Foz) between April and June of 2013. The objective of this work was on one hand to recruit new asthmatics for the study and on the other, to re - evaluate some patients diagnosed from the previous year. The study consisted of five questionnaires, which included: The Questionnaire of the knowledge of Asthma (QCSA); The Questionnaire of Morisky-Green, Asthma Control Test (ACT); Asthma and Allergic Rhinitis Control Test (CARAT). After filling the questionnaires a spirometer was done using a Vital Graph COPD – 6, determining the parameters FEV1, FEV6 and FEV1/FEV6 prebronchodilation and observing inhaling techniques. Findings: In the nine new patients, the number of correct answers to QCSA varied between 13 and 22, with only one following complete treatment. Three showed a good 7 CARAT result, five didn’t reveal any health problems, four evaluated their asthma control in the last four weeks as well controlled and five were more or less controlled according to ACT. At the time of the interview all the patients showed FEV1 scores superior to 80% and three patients did not correctly perform the inhaling technique. In the two re - evaluated patients, one patient continued to perform the inhaling technique incorrectly, though presented better results in the FEV1 and better classification in the QCSA. However, they had inferior results in the remaining questionnaires. The other patient continued to perform the inhaling technique correctly and also to forget to take his medication. The patient therefore, showed superior results in the QCSA but inferior ones in the remaining questionnaires. Conclusion: The data obtained in this study reveals important information about the needs of asthmatic patients. The findings show that the intervention of a community pharmacist is necessary to increase the degree of knowledge of patients about the illness, increase the use of therapeutics and explain the inhaling technique, this way allowing control of the illness and improved quality of patients’ lives.
