Abstract: O Sistema Renina-Angiotensina-Aldosterona é um sistema dinâmico e complexo que desempenha um papel importante na homeostase cardiovascular, equilíbrio hidroeletrolítico e funções celulares. Este sistema tem sido amplamente estudado e novos peptídeos foram ultimamente descobertos, alguns dos quais com funções opostas às da angiotensina II. Esta particularidade levanta questões sobre a organização do Sistema Renina-Angiotensina- Aldosterona e sobre a existência de um sistema contrarregulador de controlo da pressão arterial. A descoberta da existência, nos tecidos, de componentes que participam nesta cascata biológica suporta a existência do Sistema Renina-Angiotensina-Aldosterona tecidular, que complementa o Sistema Renina-Angiotensina-Aldosterona plasmático. A hipertensão arterial é um fator de risco modificável para as doenças cardiovasculares. O objetivo do tratamento da hipertensão arterial passa pela redução da pressão arterial, com redução do risco cardiovascular e melhoria da qualidade de vida. O Sistema-Renina- Angiotensina-Aldosterona está relacionado com a etiologia da hipertensão, tanto na sua origem, como na sua progressão. Desta forma, o tratamento da hipertensão arterial passa pelo bloqueio e modulação deste sistema, sendo tradicionalmente utilizados os modificadores do eixo renina-angiotensina, como os inibidores da enzima de conversão da angiotensina, os antagonistas dos recetores da angiotensina e os inibidores diretos da renina. Tendo em conta que o tratamento anti-hipertensor nem sempre atinge os objetivos terapêuticos pretendidos, torna-se crucial desenvolver novos fármacos que, em monoterapia ou em combinação com os já existentes, permitam uma terapêutica segura, que reduza eficazmente a pressão arterial e promova a proteção dos órgãos-alvo. A investigação sobre novos tratamentos para a hipertensão arterial com enfoque no Sistema Renina-Angiotensina-Aldosterona passa pela otimização da terapêutica clássica e pela descoberta de novos fármacos que atuem sobre os novos peptídeos descobertos e, que apresentando funções opostas às da angiotensina II, exercem efeito reno e cardioprotetor. Ao longo da monografia, irão ser apresentados os novos elementos do Sistema Renina- Angiotensina-Aldosterona considerados mais relevantes no controlo da hipertensão arterial e os avanços farmacoterapêuticos.
The Renin-Angiotensin System is a complex and dynamic system that plays an important role in cardiovascular and electrolyte homeostasis as well in several cellular functions. This system has been largely studied and new peptides have been discovered lately, some of them with opposite functions of angiotensin II. This particularity raises questions about the system organization and the existence of a new regulatory system which controls blood pressure. The discovery of the existence of several components of the Renin-Angiotensin System in the tissues, supports the idea that there is a tissue Renin-Angiotensin System which complements the plasmatic Renin-Angiotensin System. Hypertension is a major modifiable risk factor for cardiovascular diseases. The objective of hypertension treatment is to reduce blood pressure and improve quality of life. The Renin-Angiotensin System is related with the etiology of hypertension and its progression and so hypertension treatment passes by the blockade and modulation of the Renin-Angiotensin System. The drugs traditionally used for this propose are the Renin-Angiotensin System modifiers, which are the angiotensin converting enzyme inhibitors, the angiotensin receptors antagonists and the direct renin inhibitors. Taking into account that hypertension treatment not always achieves the desirable therapeutic goals, becomes crucial develop new drugs that in monotherapy or in association with others that allow a safe therapy in the reduction of blood pressure and in the protection of target organs as well the improvement of the quality of life. The researches for new hypertension treatments in the Renin-Angiotensin System passes by the optimization of the classical therapeutics and the discovering of new drugs which act in the newly discovered peptides that have opposite functions of angiotensin II and promote renoprotection and cardioprotection. In this monograph it is presented the new elements of the Renin-Angiotensin System which are considered more relevant for blood pressure control and the pharmacological and therapeutic advances.
