dc.description.abstractA monitorização da terapêutica medicamentosa (TDM, do inglês Therapeutic Drug Monitoring) conjuga três componentes fundamentais: a avaliação das concentrações séricas de fármacos, a análise de parâmetros farmacocinéticos (PK) e farmacodinâmicos (PD) e a interpretação clínica dos resultados, com base na análise fisiopatológica do doente em causa. Esta prática visa, sobretudo, a individualização da farmacoterapia, atendendo às especificidades de cada doente e de cada fármaco, garantindo que é o doente em si que está a ser tratado e não apenas o resultado laboratorial. Divide-se, genericamente, em três etapas fundamentais: fase pré-analítica na qual o médico realiza conscienciosamente a requisição de monitorização do fármaco, após interpretação da situação clínica e do fármaco em causa, podendo até ser esta requisição sugerida pelo farmacêutico; fase analítica na qual se procede à colheita e análise laboratorial da amostra plasmática, em intervalos de tempo pré-definidos, com particular ênfase na determinação da concentração do fármaco; e a fase de interpretação farmacocinética, tendo em conta os dados clínicos do doente para que haja uma intervenção favorável na decisão clínica. O contributo do farmacêutico hospitalar, nestas diferentes etapas, afigura-se essencial para a obtenção de resultados aplicáveis e, consequentemente, para o aumento da sobrevida do doente. No sentido de rentabilizar recursos, a TDM aplica-se a fármacos cuja concentração plasmática se relaciona com os efeitos farmacológicos (terapêuticos e/ou laterais) e que são dotados de características que dificultam grandemente a uniformização da posologia. Esta monografia centra-se na descrição da monitorização da terapêutica medicamentosa, desde os requisitos básicos até à sua concretização na prática clínica, no caso particular da gentamicina, passando por todas as etapas necessárias e enfatizando o papel do farmacêutico hospitalar para o sucesso desta metodologia. A gentamicina é um antibiótico largamente utilizado no tratamento de infeções bacterianas severas, particularmente em unidades de cuidados intensivos. Pela margem terapêutica estreita que apresenta, pelo seu potencial oto e nefrotóxico e pelas resistências adquiridas pelos principais microrganismos responsáveis pelas infeções nosocomiais, a TDM afigura-se uma prática singular para o sucesso desta terapêutica.pt
dc.description.abstractTherapeutic Drug Monitoring (TDM) assembles three fundamental components: the assessment of drug’s concentrations in serum, the analysis of pharmacokinetics (PK) and pharmacodynamics (PD) profiles and the clinical interpretation of the results, based on the pathophysiological conditions of the patient. This practice is aimed mainly at the individualization of pharmacotherapy, meeting the specific needs of each patient and each drug, ensuring that the focus is the patient and not only laboratory results. Generally, TDM includes three main stages: the pre-analytical phase in which the physician conscientiously performs the request, taking into account the clinical situation and the drug in question, and this request may even be suggested by the hospital pharmacist; the analytical phase in which plasma (or serum) samples are collected at predefined times postdosing and subsequently analyzed to determine drug’s concentration; and, finally, the phase of pharmacokinetics interpretation, taking into account the clinical data of the patient so that there is a favorable clinical intervention in decision. The contribution of the hospital pharmacist, in different stages, is essential to achieve relevant results and increase the patient survival. In order to capitalize resources, TDM is employed to drugs with narrow therapeutic range, displaying a well-known relationship between drug plasma concentrations and pharmacological effects (therapeutic and/or lateral) and that exhibit characteristics that greatly difficult the application of the same dosage regimen, independently of the patient. This work focuses on the description of TDM from the basic requirements to their implementation in clinical practice, particularizing the gentamicin case, describing all the required steps and emphasizing the role of the hospital pharmacist for the success of the therapeutic monitoring of gentamicin. Gentamicin is an antibiotic widely used to treat severe bacterial infections particularly in intensive care units. Given its narrow therapeutic index, its ototoxic and nephrotoxic potential and the development of resistance mechanism by the main microorganisms responsible for nosocomial infections, the TDM is currently a singular practice for the success of this therapy.pt
