Cuidados Farmacêuticos no Tratamento do Doente Oncológico

Abstract

Oncology has been assuming a growing importance in any healthcare system, mostly due to the increasingly incidence of cancer worldwide at which corresponds an increase in the number of treatments, usually involving chemotherapeutic agents. This fact is setting pressure among the healthcare professionals to become more efficient and productive, demanding thus more attention and a more prominent role from the pharmacist in the treatment of cancer patients. Therefore, this monography aims to analyze the pharmaceutical care during the treatment of cancer patients. To achieve this objective, a three-step analysis was carried out encompassing the duties and responsibilities of an oncology pharmacist within United States of America (USA), an observational study performed in the Portuguese Institute of Oncology-Porto, and, finally, an evaluation of the possibility to decentralize the ambulatory oral therapeutic prescriptions in Portugal. Contrarily to the current situation in Portugal, the USA pharmacist is capacitated with specific training in oncology derived from their specialization in oncology pharmacy; thereby, the pharmacist plays an important role in the treatment of cancer patients, and accounts with major responsibilities in clinical pharmacy. Indeed, the observational study revealed that in Portugal, the key responsibilities of the pharmacist are still limited to their traditional non cognitive functions, and that there is a lack of complementary training in this field. On the other hand, the dispensing of oral therapy should be decentralized, probably through community pharmacies. In summary, there are major differences between USA and Portugal in what regards the pharmacist’s role in the treatment of cancer patients; however, new actions to overcome these differences are expected in the future by promoting a change in the current paradigm in Portugal, foreseeing a more dynamical and interventive pharmacist.

A oncologia tem-se vindo a revelar uma área extremamente sensível em qualquer sistema de saúde, na qual o aumento da incidência de cancro tem conduzido a um aumento do número de tratamentos, geralmente envolvendo quimioterapia. Este facto tem colocado alguma pressão junto das equipas de saúde no sentido de se tornarem mais produtivas e eficientes, exigindo mais atenção e dedicação bem como um papel mais relevante e interventivo do farmacêutico no tratamento do doente oncológico.Desta forma, a presente monografia tem por objetivo analisar os cuidados farmacêuticos desempenhados ao longo do tratamento do doente oncológico. Efetuou-se uma análise tripartida que engloba: o modelo de atuação do farmacêutico no domínio oncológico nos Estados Unidos da América (EUA); um estudo observacional levado a cabo no Instituto Português de Oncologia do Porto FG, EPE; e uma abordagem à descentralização da dispensa da terapêutica oncológica oral em Portugal, tendo por base o modelo dos EUA, e os principais problemas detetados na sua descentralização.Dotado de uma formação diferenciada em oncologia, proveniente da especialização em oncologia farmacêutica, o farmacêutico nos EUA apresenta uma contribuição extremamente importante no tratamento dos doentes, com funções alargadas no domínio de farmácia clínica, ao contrário do que se verifica em Portugal. De facto, o estudo observacional revelou que as funções do farmacêutico ainda estão algo limitadas às funções tradicionais, não clínicas, e, simultaneamente, persistem as lacunas associadas à inexistência de formação complementar especializada nesta área. A dispensa da terapêutica oncológica oral em Portugal deverá ser descentralizada a curto prazo, através da farmácia comunitária.Ao nível do tratamento oncológico, as diferenças das funções exercidas pelo farmacêutico nos EUA e em Portugal ainda são notórias, mas as perspetivas futuras passarão por tentar atenuar estas diferenças e, promover a evolução das funções do farmacêutico em Portugal, para uma atividade mais cognitiva.
