Abstract: Primary and secondary prevention of cardiovascular diseases is currently a major concern of health organizations. This prevention involves essentially the reduction of the plasma levels of low density lipoprotein cholesterol (c-LDL), but the hypocholesterolemic drugs, mostly statins, did not cover all types of patients.With the discovery of proprotein convertase subtilisin/kexin type 9 (PCSK9), a promising target was identified by reducing the risk of development of atherosclerosis. This enzyme, when bound to low density lipoprotein receptors (r-LDL), will induce the degradation of these receptors, preventing them from being recycled and expressed again on the surface of the hepatocyte. Thus, there will be a lower uptake of c-LDL from the plasma, what increase its levels and leads to an accumulation in the inner walls of the arteries, developing atherosclerosis. With the discovery of the mechanism of action of PCSK9, it was observed that the inhibition of this enzyme can increase the expression of r-LDL, inducing the decrease of levels of c-LDL.After observing the mechanism of action of PCSK9, several pharmacological approaches were developed for the inhibitors of this enzyme, the most successful were the monoclonal antibodies. Two antibodies, evolocumab and alirocumab, were studied in detail during several clinical trials in which the efficacy of these two drugs was shown to reduce plasma levels of c-LDL, achieving reductions up to 70%. The advantage of these drugs is that they are effective in those types of patients that are not covered by statins, namely, in patients with homozygous and heterozygous familiar hypercholesterolemia, patients that are intolerants to statins and patients who even with the maximum statin dosage did not reach acceptable levels of c-LDL. In addition, both antibodies were found to be safe as well properly tolerated, and any serious adverse reactions were reported.In 2015 the European Medicines Agency approved the commercialization of two monoclonal antibodies, Praluent® (alirocumab) and Repatha® (evolocumab). These drugs are indicated for situations in which statins are not sufficient and for homozygous hypercholesterolemia familiar. It is clear that the introduction of these drugs into hypocholesterolemic therapies will lead to very beneficial results and will cover the whole population. However, it is necessary in the future to find analogues of these drugs at a more affordable price so that it is available to all.This document is divided into two parts, a first that include the monograph about the inhibitors of PCSK9 and another dedicated to a report of an internship that I have done in community pharmacy.
A prevenção primária e secundária das doenças cardiovasculares é atualmente uma das maiores preocupações das organizações de saúde. Esta prevenção passa essencialmente por reduzir os teores plasmáticos do colesterol das lipoproteínas de baixa densidade (c-LDL). No entanto, as terapias hipocolesterolemiantes convencionais, principalmente as estatinas, não abrangem todos os tipos de doentes. Com a descoberta da ação da pro-proteína convertase subtlisina/kexina tipo 9 (PCSK9), detetou-se um alvo promissor na redução do risco de desenvolver aterosclerose. Esta enzima ao ligar-se aos recetores das lipoproteínas de baixa densidade (r-LDL), vai induzir a degradação destes, impedindo que sejam reciclados e expressos novamente na superfície do hepatócito. Assim, vai haver uma menor captação do c-LDL do plasma, levando a um aumento dos seus teores e a acumulação na camada íntima das artérias, desenvolvendo aterosclerose. Com a descoberta do mecanismo de ação da PCSK9, percebeu-se que a inibição desta enzima aumentava a expressão dos r-LDL, induzindo a diminuição dos teores plasmáticos do c-LDL. Após a observação do mecanismo de ação da PCSK9 foram desenvolvidas várias abordagens farmacológicas para os inibidores desta enzima, sendo que a mais bem sucedida foi os anticorpos monoclonais. Foram estudados em detalhe dois anticorpos, o evolocumab e o alirocumab e durante vários ensaios clínicos foi notória a eficácia destes dois fármacos na redução do c-LDL plasmático, atingindo reduções até 70%. A vantagem destes fármacos é que são eficazes nos tipos de doentes que não são abrangidos pelas estatinas, nomeadamente em doentes com hipercolesterolémia familiar homozigótica e heterozigótica, indivíduos intolerantes às estatinas e doentes que mesmo com a máxima dosagem de estatina não alcançavam níveis de c-LDL aceitáveis. Além disso, ambos anticorpos revelaram-se seguros e bem tolerados, não tendo sido reportadas reações adversas graves.Em 2015 foi aprovada pela European Medicines Agency a comercialização de dois anticorpos monoclonais, o Praluent® (alirocumab) e o Repatha® (evolocumab). Estes fármacos estão indicados para as situações em que as estatinas não são suficientes e para a hipercolesterolémia familiar. É evidente que a integração destes fármacos nas terapias hipocolesterolemiantes vai levar a resultados muito benéficos e abranger toda a população. No entanto, é necessário no futuro encontrar análogos destes fármacos a um preço mais acessível para que esteja disponível para todos.Este documento encontra-se dividido em duas partes, uma primeira com a monografia sobre os inibidores da PCSK9 e uma segunda dedicada ao relatório de estágio que realizei em farmácia comunitária.
