Abstract: Atualmente, as doenças cardiovasculares são uma das maiores causas de mortalidade a nível mundial, sendo o aumento de colesterol total sérico, sobretudo o colesterol LDL, o principal fator de risco ao desenvolvimento destas doenças. Assim, está demonstrado que a redução deste parâmetro reduz a incidência destas patologias em indivíduos com dislipidemia assim como as mortes relacionadas com as mesmas. Na Europa, no que respeita aos casos de hipercolesterolemia, é recomendada como primeira abordagem medidas de alteração do estilo de vida, e caso não sejam verificados decréscimos significativos nos níveis de colesterol são utilizadas como primeira linha farmacológica as estatinas. Contudo, esta classe de fármacos nem sempre é bem tolerada pelos pacientes pelo que, para além das ofertas da medicina tradicional, tem-se verificado um crescimento das terapias alternativas bem como um maior interesse por parte do público. Apesar da evidência relativa à eficácia destes produtos ainda ser pouco concreta e a regulação deste mercado ser confusa, este é um setor que tem tido um crescimento exponencial nos últimos anos sendo um desafio à prática médica. O arroz fermentado por Monascus purpureus está incluído nas terapias naturais de redução de lípidos e tem demonstrado eficácia, tolerabilidade e segurança em indivíduos intolerantes a estatinas. O presente trabalho tem como principais objetivos a perceção do impacto das doenças cardiovasculares a nível Europeu e Nacional assim como dos seus fatores de risco; perceber a relação entre hipercolesterolemia e processo aterosclerótico; abordar a importância das estatinas na hipercolesterolemia e as suas limitações; analisar a evidência atual sobre a eficácia e segurança do arroz fermentado por M. purpureus na hipercolesterolémia; e por fim, abordar a introdução de Coenzima Q10 nos produtos à base de Red Yest Rice.
Nowadays, cardiovascular diseases are one of the major causes of mortality worldwide being the increase of the serum cholesterol total, especially LDL cholesterol, the main factor of risk for the development of these diseases. Thus, it has been shown that the reduction of these parameters decreases the incidence of these pathologies in individuals with dyslipidemia as well as the deaths related to them. In Europe, in the case of hypercholesterolemia, a first approach to measures of lifestyle changes is recommended and if there are no significant decreases in cholesterol levels, statins are used as the first pharmacological line. However, this class of drugs isn’t always well tolerated by patients, so that in addition to the traditional medicine offerings, there has been an increase in alternative therapies as well as more public interest. Although the evidence regarding the effectiveness of these products is still unclear and the regulation of this market isn’t very well established, this is a sector that has experienced exponential growth in recent years and is a challenge to medical practice. Monascus purpureus fermented rice is included in natural lipid-lowering therapies and has demonstrated efficacy, tolerability, and safety in individuals who are intolerant to statins. The main objective of this monography is to understand the impact of cardiovascular diseases at a European and National level as well as their factors of ricks; to understand the relationship between hypercholesterolemia and atherosclerotic process; discuss the importance of statins in hypercholesterolemia and its limitations; to analyze the current evidence of the efficacy and safety of M. purpureus fermented rice in hypercholesterolemia; and finally, to address the introduction of Coenzyme Q10 in Red Yest Rice based products.
