Abstract: Após a aprovação do primeiro inibidor do checkpoint imunitário, a imunoterapia para o cancro tem sido alvo de uma forte ascensão, com o desenvolvimento de diversas estratégias que intervêm na resposta imune anti-tumoral, sendo, hoje, considerada como um dos pilares da terapêutica para o cancro. O sucesso da imunoterapia tem tido como principais responsáveis os inibidores do checkpoint imunitário e distingue-se pela indução de respostas com uma grande durabilidade, para diversos tipos de tumores. Contudo, estes benefícios clínicos não se observam em todos os doentes, fruto da existência de vários mecanismos de resistência inata e adaptativa, que impedem uma resposta inicial à imunoterapia, ou de resistência adquirida, desenvolvida após uma resposta inicial e que conduz à recidiva do doente. Com o intuito de vencer as resistências do tumor ou do seu microambiente à imunoterapia, a imunoterapia combinada preconiza a combinação sinérgica de uma imunoterapia, geralmente um inibidor do checkpoint imunitário, com outra imunoterapia ou com outro tipo de terapêutica (quimioterapia, radioterapia, entre outras), para atuar em diferentes etapas do ciclo cancro-sistema imunitário e, assim, melhorar a sobrevivência de um maior número de doentes. Nesta monografia, irei expor os principais fatores que podem contribuir para a resistência à imunoterapia, qual o raciocínio subjacente às combinações de inibidores do checkpoint imunitário com outras estratégias terapêuticas, quais as combinações já aprovadas e abordarei, ainda, a necessidade da determinação de biomarcadores para a seleção da melhor imunoterapia ou combinação para cada caso, assim como o estabelecimento de parcerias entre empresas resultante do investimento empregue na área da imunoterapia combinada, que se apresenta como o futuro da imunoterapia para o cancro.
Since the approval of the first immune checkpoint inhibitor, the cancer immunotherapy has grown strongly with the development of different strategies that act in the cycle cancer-immunity, and, today is considered, as one of the pillars of cancer care. The immune checkpoint inhibitors are the mainly responsible for the success of cancer immunotherapy, which can induce durable responses for a large range of tumour types. Despite of this clinical benefits, not all the patients can achieve them due to mechanisms of innate and adaptive resistance that prevent an initial response to the immunotherapy, or due to mechanisms of acquired resistance that lead to relapse of patients after a period of response. In order to overcome the tumour or the tumour microenvironment resistances, combined immunotherapy advocates the synergic combination between an immunotherapy, generally an immune checkpoint inhibitor, with another immunotherapy or with another cancer therapy (such as chemotherapy, radiotherapy, etc.), to act at different points of the cancer-immunity cycle and, therefore, to improve the survival of a major number of patients.In this monograph, I will expose the main factors that contribute to the resistance to immunotherapy, what is the rational underlying the immune checkpoint inhibitor combinations with other therapeutic strategies, what are the combined immunotherapies that are already approved, and I will also refer the crescent need of determine several biomarkers with the aim of selecting the best immunotherapy or combination for each individual case, as well as the establishment of partnerships between companies as a result of the investment made in the combined immunotherapy area, which is currently seen as the future of the cancer immunotherapy.
