Abstract: A prevalência da diabetes mellitus (DM) tem vindo a aumentar mundialmente e, embora já tenha sido alvo de muita investigação, a sua relação com a disfunção macrovascular permanece pouco clara. Isto deve-se em particular à presença de múltiplas outras comorbilidades nestes doentes, também elas potenciadoras do risco vascular.Neste estudo foram selecionados 80 doentes diabéticos que foram monitorizados regularmente em meio hospitalar durante pelo menos 3 anos, com avaliação de parâmetros alvo em cada visita. Findo o seguimento, subdividiram-se em 4 grupos dependentes entre si, mas independentes em pares, dentro dos quais se realizou a análise estatística dos dados. Usaram-se os testes de Mann-Whitney, ANOVA a dois fatores e Friedman assim como uma regressão logística, que permitiram relacionar as oscilações das variáveis em estudo com a ocorrência de eventos macrovasculares e também estabelecer relações de causalidade. As análises foram realizadas no SPSS, versão 23, e avaliadas ao nível de significância de 5%.Observou-se uma relativa homogeneidade entre os grupos formados, excetuando a presença de antecedentes de enfarte agudo do miocárdio (EAM) e de acidente vascular cerebral (AVC) prévios dentro dos grupos com complicações cardiovasculares ou cerebrovasculares, respetivamente. Dentro dos grupos identificou-se um aumento dos valores de tensão arterial diastólica (TAD), frequência cardíaca (FC) e creatinina e uma diminuição do peso e índice de massa corporal relevantes apenas no grupo onde ocorreram complicações cerebrovasculares. As restantes variáveis mantiveram-se relativamente estáveis durante o estudo não existindo dessa forma variações estatisticamente significativas. Estabeleceram-se relações de causalidade entre a presença de EAM prévio e valores baixos de FC com a ocorrência de complicações cardiovasculares e entre a presença de AVC prévio e valores altos de tensão arterial sistólica (TAS) e a ocorrência de complicações cerebrovasculares. Posto isto, foi possível identificar os fatores de risco mais predisponentes para ambos os tipos de complicações, nomeadamente a nível de fatores não modificáveis, como os antecedentes de complicações prévias, que se demonstrou elevarem o risco independentemente da presença de outras comorbilidades. Adicionalmente percebeu-se que, embora se identifiquem fatores de risco mais preponderantes que outros, designadamente a TAS a nível cerebrovascular, é a interação entre múltiplos fatores e o risco cumulativo que tal comporta que está na base da ocorrência de muitos destes eventos macrovasculares. Conclui-se que, mesmo em doentes com uma patologia tão dominante como a diabetes mellitus, é essencial uma abordagem médica holística visando não só prevenir eventos, como manter um controlo adequado dos múltiplos fatores de risco que sejam identificados, ao invés do controlo agressivo de um fator isoladamente.
The prevalence of diabetes mellitus has been steadily increasing worldwide and despite having been the target of several studies, its relation with macrovascular dysfunction remains nuclear. This is thought to be mainly due to the presence of other accompanying diseases, which themselves account for a higher vascular risk. This study selected 80 diabetic patients, which were regularly monitored in a hospital environment for up to 3,5 years during which several variables were evaluated. Following this, each subject was distributed within 4 groups, independent in pairs, to facilitate the ensuing statistical analysis. The Mann-Whitney, two-way ANOVA and Friedman tests were applied, along with a logistic regression, which allowed for correlation between variables and the occurrence of macrovascular events, as well as the identification of causality factors. Analysis were performed with SPSS, version 23, and were evaluated at a 5% significance level.The study group distribution was generally uniform between the 4 groups, with the exception being the pre-existing history of myocardial infarction and stroke within the group cardiovascular or cerebrovascular events occurred in, respectively. The data analysis revealed a correlation between higher diastolic blood pressure, heart rate and creatinine values, as well as lower weigh and body mass index and the occurrence of cerebrovascular events. The remaining variables maintained steady values throughout the study and where therefore not statistically significant. A causal relation was identified between pre-existing history of myocardial infarction and low heart rate with the incidence of cardiovascular events, and also between pre-existing history of stroke and elevated systolic blood pressure, both with the occurrence of cerebrovascular events.As such, these results allowed for the identification of preponderant risk factors for both types of macrovascular complications targeted in this study, namely unmodifiable risk factors, such as previous medical history of such events, acting independently of other coinciding modifiable risk factors. Additionally, some factors were found to be more relevant than others, namely systolic blood pressure on the incidence of stroke, however it was the interaction of several uncontrolled risk factors that was found to be the basis for the occurrence of these complications. All in all, this study revealed that even in patients with such a predominant disease such as diabetes, it’s essential to take up a wholesome approach with the intent of not only preventing vascular events, but also of keeping an adequate control of multiple risk factors as opposed to aggressively attempting to lower a single one.
