Abstract: RESUMO Introdução: Dada a prevalência crescente da diabetes mellitus tipo 2 (DM2) com impacto na morbimortalidade da população, e considerando a problemática da não adesão terapêutica enquanto problema de saúde pública, torna-se importante avaliar a relação entre a prevalência de adesão terapêutica em DM2 e os conhecimentos do doente sobre a doença. Métodos: Estudo descritivo correlacional de uma amostra por conveniência da população com DM2 com consultas na Unidade de Cuidados de Saúde Personalizados do Centro de Saúde Norton de Matos, em Coimbra. O instrumento de colheita de dados incluiu a caracterização sociodemográfica e clínica, o Questionário dos Conhecimentos da Diabetes (QCD), a Escala de Atividades de Autocuidado com a Diabetes (EAACD) e a Medida de Adesão aos Tratamentos (MAT) para avaliar a adesão terapêutica indireta e direta, respetivamente. O controlo glicémico foi avaliado através da HbA1c.Resultados: No total dos 35 doentes com DM2 em estudo, 54,29% eram do sexo masculino, com idade média de 68,26 anos, sem escolaridade (28,57%) ou com escolaridade até ao 6º ano (37,14%), diagnóstico de DM2 em média há 11,34 anos, medicados com antidiabéticos orais (57%). 90% dos doentes demonstraram conhecimentos corretos sobre a DM2, enquanto 3% e 7% conhecimentos incorretos e desconhecidos, respetivamente. Em relação à adesão terapêutica avaliada pela EACCD, a atividade de autocuidado “medicamentos” apresentou maior média de adesão (6,48 dias). Relativamente à MAT, a média de adesão terapêutica avaliada foi 80,25%. 21 doentes com DM2 (60%) apresentaram um controlo glicémico avaliado pela HbA1c <7%.Discussão: Os conhecimentos do doente sobre a DM2 correlacionam-se fortemente com o controlo glicémico e com a adesão terapêutica indireta e direta, estando esta última moderadamente correlacionada com o controlo glicémico (tratamento mais exigente/HbA1c <7%). Também existe diferença estatisticamente significativa entre a adesão terapêutica e o tratamento, sendo o grau de adesão terapêutica maior no tratamento com antidiabéticos orais. O índice de massa corporal e o perímetro abdominal variam inversamente com a adesão terapêutica.Conclusão: Urge reconhecer os conhecimentos do doente sobre a DM2 na promoção da adesão terapêutica. Sugerimos a educação para a saúde em todas as fases do tratamento.Palavras-Chave: Diabetes mellitus tipo 2; adesão terapêutica; conhecimentos na diabetes.
ABSTRACT Introduction: Given the growing prevalence of Diabetes mellitus type 2 (DM2) with an impact on the mortality and morbidity of the population, and considering the problem of non-adherence therapeutic as a public health problem, it becomes important to calculate the relation between the prevalence of therapeutic adherence in DM2 and the knowledge of the patient regarding the disease.Methods: Descriptive correlational study of a sample by convenience of the population with DM2 with appointments in the Unidade de Cuidados de Saúde Personalizados do Centro de Saúde Norton de Matos, in Coimbra. The instrument used for data collection includes the socio-demographic characterization and clinic, the Diabetes Knowledge Questionnaire (DKQ), the Diabetes Self-Care Activities Scale (DSCAS) and the Measure Treatment Adherence (MTA) to evaluate the therapeutic adherence, direct and indirect, respectively. The glycemic control was done through the measurement of HbA1c. Results: In a total of 35 patients with DM2 in the study, 54,29% were male, with an average age of 68,26 years, without schooling (28,57%) or with schooling up to the sixth grade (37,14%), diagnosed with DM2 on average 11,34 years ago, medicated with oral anti diabetic medication (57%). 90% of the patients showed correct knowledge about DM2, while 3% and 7% showed incorrect knowledge or unknown, respectively. In relation with the therapeutic adherence evaluated by the DSCAS, the activity of self-care “medication” showed the best average of adherence (6,48 days). Relating to MTA, the average of therapeutic adherence was calculated at 80,25%. 21 patients with DM2 (60%) presented a glycemic control showed by HbA1c <7%.Discussion: The patients knowledge about DM2 is strongly related with the glycemic control and direct and indirect therapeutic adherence, being the latter moderately correlated with the glycemic control (more demanding control/HbA1c <7%). There is also a significantly statistical difference between the therapeutic adherence and treatment, being the degree of therapeutic adherence higher in the treatment with oral anti diabetic medications. The body mass index and the abdominal perimeter vary inversely with the therapeutic adherence.Conclusion: It urges to recognize the knowledge of the patient about DM2 in the promotion of therapeutic adherence.
