Abstract: Introduction: It is known that in recent years the geriatric population, chronic diseases and the average life expectancy in Portugal have been increasing thanks to the evolution in quality and accessibility of health care. It is therefore reasonable to assume that, with the increase in the number of elderly and dependent patients, there is a greater need for care, both by health professionals, the so called formal caregivers, and by informal caregivers.Caring for a dependent person while trying to ensure the maximum quality of life and postponing the use of institutionalization can be very rewarding, it is something that is done with feeling and respect. However, it often happens that the informal caregiver is not prepared, both physically and mentally as well as formatively, to deal with the burden that this caring activity can present and gradually enter into burnout.Burnout is a syndrome characterized by exhaustion and physical, mental and emotional distress, which can have serious consequences for the caregiver's health, negatively affecting the attitude and quality of the care given to the dependent individual.The aim of this article is to make a sociodemographic characterization the of the primary informal caregivers and to evaluate which ones present greater risk for the development of burnout syndrome. Finally, we will try to identify coping strategies used by caregivers.Methods: Realization of a narrative review of the bibliography on this theme. The selection of articles for this review was performed using the PubMed / MEDLINE database, applying a 5-year time limit (January 2013 to January 2018). For this purpose, combinations of the following terms were used: ‘burnout’, ‘caregivers’, ‘coping’, ‘depersonalization’, ‘burden’, ‘anxiety’, ‘fatigue’, ‘depression’, ‘stress’. Articles were searched in portuguese and english.Reference textbooks and other electronic resources, such as the Clinical Key and the Web of Science, were also consulted.Results: It was found that most of the time the caregivers were female, they were the patient's spouse and they were between the ages of 46 and 81 years. Analyzing the sociodemographic characteristics, several associations were established with the mental health status of the caregiver.Of all the articles surveyed, only four focused specifically on informal caregiver burnout. The most relevant results showed a direct relationship between the higher levels of burnout and: the greater number of hours spent in care; the lowest income; negative beliefs about the impact of the disease on the informal caregiver; when the informal caregiver had alexithymia as the dominant personality trait and when the patient was more dependent and reported lower quality of life.Regarding the coping strategies, it was verified that the adopted strategy depended on the personality and sociodemographic characteristics of the caregiver, their psychological resources, the severity and stage of the disease and the degree of dependence of the patient. Most informal caregivers used emotion-centered coping more frequently, but it was often considered the least effective because it was associated with increased psychiatric morbidity.Conclusion: Informal caregivers are increasingly needed to ensure the provision of health care, emotional support and quality of life for the elderly and dependent patients in a family setting. However, given that their function usually runs for long periods of time, caregivers put themselves at great risk of psychiatric and psychological morbidity which can culminate in burnout, affecting their own lives and that of the patient.To deal with the situation, often unconsciously, the caregiver resorts to coping strategies that may not be the most appropriate, or the most effective, to reduce the stress and burden of care. Health professionals should try to identify informal caregivers who are at greater risk of burnout in order to act preventively and provide information on coping strategies that are useful and effective in dealing with the situation so that the caregiver remains healthy.
Introdução: É conhecido que nos últimos anos a população geriátrica, as doenças crónicas e a esperança média de vida em Portugal têm vindo a aumentar graças há evolução, em qualidade e acessibilidade, dos cuidados de saúde. É, portanto, lógico admitir que, com o aumento do número de idosos e doentes dependentes, há uma maior necessidade de prestação de cuidados, tanto por profissionais de saúde, também denominados cuidadores formais, como por cuidadores informais. Cuidar de uma pessoa dependente enquanto se tenta garantir o máximo de qualidade de vida e o adiamento do recurso a institucionalizações pode ser muito gratificante, é algo que se faz com sentimento e respeito. No entanto, acontece que, muitas vezes o cuidador informal não está preparado, tanto a nível físico e mental como a nível formativo, para lidar com a sobrecarga que esta atividade de cuidar pode apresentar e gradualmente entrar em burnout.O burnout é uma síndrome que se caracteriza por exaustão e angústia física, mental e emocional, que pode ter consequências graves para a saúde do cuidador, repercutindo-se negativamente na atitude e na qualidade do cuidado que é prestado ao indivíduo dependente.O objetivo deste artigo é fazer a caracterização sociodemográfica dos cuidadores informais primários e procurar avaliar quais apresentam maior risco para o desenvolvimento de síndrome de burnout. Por fim, procurar-se-á identificar as estratégias de coping utilizadas pelos cuidadores. Métodos: Realização de uma revisão narrativa da bibliografia sobre esta temática. A seleção de artigos para esta revisão, foi realizada com recurso à base de dados PubMed/MEDLINE, aplicando um limite temporal de 5 anos (janeiro de 2013 a janeiro de 2018). Para o efeito, foram utilizadas combinações dos seguintes termos: ‘burnout’, ‘caregivers’, ‘coping’, ‘depersonalization’, ‘burden’, ‘anxiety’, ‘fatigue’, ‘depression’, ‘stress’. Foram pesquisados artigos em português e inglês. Foram também consultados livros de texto de referência e outros recursos eletrónicos como a Clinical Key e a Web of Science.Resultados: Verificou-se que na maioria das vezes os cuidadores eram do sexo feminino, eram o cônjuge do doente e tinham uma idade compreendida entre os 46 e os 81 anos. Analisando as características sociodemográficas, foram estabelecidas várias associações com o estado de saúde mental do cuidador.De todos os artigos pesquisados apenas quatro se debruçavam especificamente sobre o burnout no cuidador informal. Os resultados mais relevantes demonstraram haver uma relação direta entre os maiores níveis de burnout e: o maior número de horas despendidas nos cuidados; o menor rendimento; as crenças negativas sobre o impacto da doença no cuidador informal; quando o cuidador informal tinha alexitimia como traço dominante de personalidade e quando o doente era mais dependente e relatava menor qualidade de vida.Sobre as estratégias de coping foi verificado que a estratégia adotada dependia da personalidade e das características sociodemográficas do cuidador, dos seus recursos psicológicos, da severidade e estádio da doença e do grau de dependência do doente. A maioria dos cuidadores informais utilizava mais frequentemente o coping centrado na emoção que, no entanto, se considerou, na maioria das vezes, o menos eficaz por estar associado a maior morbilidade psiquiátrica. Conclusão: Os cuidadores informais são, cada vez mais, imprescindíveis para garantir a prestação de cuidados de saúde, apoio emocional e qualidade de vida aos idosos e doentes dependentes num seio familiar. No entanto, tendo em conta que a sua função geralmente decorre por longos períodos de tempo, os cuidadores colocam-se em grande risco de morbilidade psiquiátrica e psicológica que pode culminar em burnout, afetando a sua própria vida e a do doente. Para lidar com a situação, muitas vezes inconscientemente, o cuidador recorre a estratégias de coping que podem não ser as mais adequadas, ou as mais eficazes, para reduzir o stress e a sobrecarga sentida pela prestação de cuidados. Os profissionais de saúde devem tentar identificar os cuidadores informais que apresentam maior risco de burnout, de modo a atuar preventivamente, e fornecer informações sobre estratégias de coping úteis e eficazes para lidar com a situação de modo a que o cuidador se mantenha saudável.
