Resumo: Introdução: Em Portugal, apesar das taxas de AM serem elevadas aquando da alta da maternidade, rapidamente decaem a valores longe dos ideais. Em 2011, os Açores registaram a taxa de AM mais baixa aos 6 meses, tomando, por isso, especial importância no estudo de determinantes preponderantes nesta região para o abandono ou não iniciação desta prática.Objetivo: Este estudo teve como objetivo primordial identificar quais as principais barreiras ao AM nos Açores.Métodos: Estudo transversal, observacional com amostra de conveniência, constituída por mães com filhos a frequentar uma IPSS da ilha de S. Miguel, que já tiveram pelo menos uma experiência de amamentação. A abordagem inicial consistiu numa sessão audiogravada de um focus group com algumas das mães na tentativa de recolher as condicionantes mais importantes que encontraram à prática da amamentação. Posteriormente, foi elaborado um questionário baseado em todas as variáveis recolhidas neste encontro e outros estudos até então realizados procedendo então à comparação do impacto de cada uma delas.Resultados: A amostra incluiu mães com uma média de idades de 32,1 ± 5,18 anos que tinham pelo menos um filho com idade inferior a 3 anos matriculado numa das IPSS selecionadas. O tempo médio de amamentação materna exclusiva foi de 2,97 ± 3,03 meses e o tempo total de amamentação de 7,49 ± 6,59 meses. 77,8% das participantes amamentou todos os filhos, 9,4% amamentou nenhum e 11,1% amamentou apenas alguns dos filhos. Os fatores que tiveram maior peso no abandono da amamentação nestas mães, no geral, foram o stress e o cansaço, bem como a produção insuficiente de leite que se repercutia no peso do bebé e probelmas mamários. Para as mães que amamentaram menos filhos o achar que o leite animal era mais benéfico para o seu filho (p=0,002) e a publicidade ao leite artificial fazer crer ser este o meio mais capaz de proporcionar uma nutrição mais eficaz (p=0,005) foram as barreiras mais apontadas. A dificuldade em conciliar com o trabalho (p=0,002), o sentir-se incapaz (p=0,038) e o achar que o leite era de fraca qualidade (p=0,029) também foram limitantes ao AM. Também o número de filhos (p=0,001) e o número de pessoas com quem coabita (p=0,021) tiveram influência negativa no número de filhos que é amamentado e no tempo de amamentação, respetivamente, traduzindo em parte uma sobrecarga para a mulher. Classes sociais mais desfavorecidas também amamentam menos filhos (p<0,001). Para mães que decidiram não amamentar de todo os fatores mais apontados foram a crença de que o leite animal era mais benéfico (p<0,001) e a publicidade ao mesmo (p<0,001), a pressão por parte da família para não amamentar (p=0,020) e ainda a falta de informação relativamente à amamentação (p=0,047).Conclusão: Foram identificados alguns fatores limitativos para a prática da amamentação nesta amostra da ilha de S. Miguel, o que poderá ser útil para traçar intervenções no sentido de melhorar esta prática.
Introduction: In Portugal, although breastfeeding (BF) rates are high at maternity leave, they rapidly decline to values ​​that are far from ideal. In 2011, the Azores had the lowest rate of BF at 6 months, and it took particular importance in the study of preponderant determinants in this region for the abandonment or non-initiation of this practice. Objective: This study had the main objective to identify the barriers to BF in Azores. Methods: A cross-sectional, observational study with a convenience sample, consisting of mothers with children attending an IPSS from the island of S. Miguel, who had at least one experience of breastfeeding. The initial approach consisted of an audiograved session of a focus group with some of the mothers in an attempt to collect the most important constraints that they found to the practice of BF. Subsequently, a questionnaire was drawn up based on all the variables collected at this meeting and other studies performed until then, comparing the impact of each one of them.Results: The sample included mothers with a mean age of 32.1 ± 5.18 years who had at least one child under the age of 3 enrolled in one of the selected IPSS. The mean duration of exclusive breastfeeding was 2.97 ± 3.03 months and the total breastfeeding time was 7.49 ± 6.59 months. 77.8% of the participants breastfed all the children, 9.4% breastfed none and 11.1% breastfed only some of the children. The factors that had the greatest impact in the abandonment of BF in these mothers, in general, were stress and tiredness, as well as insufficient milk production that had repercussions on the baby's weight and breast problems. For mothers who breastfed less children, the belief that animal milk was more beneficial to their child (p = 0.002) and advertising to artificial milk making them think that it was the most effective way to provide more effective nutrition to their child (p = 0.005) were the most pointed barriers. The difficulty in reconciling with work (p = 0.002), feeling incapable (p = 0.038) and finding that milk had poor quality (p = 0.029) were also limiting to BF. The number of children (p = 0.001) and the number of people with whom they cohabited (p = 0.021) also had a negative influence on the number of children who are breastfed and on the time of breastfeeding, respectively, which translates into an overload for the woman. More disadvantaged social classes also breastfeed fewer children (p <0.001). For mothers who decided not to breastfeed the factors most pointed were the belief that animal milk was more beneficial (p <0.001) and advertising (p <0.001), family pressure not to breastfeed (p=0.020) and lack of information regarding breastfeeding (p = 0.047). Conclusion: Some limiting factors for the practice of breastfeeding were identified in this sample of the island of S. Miguel, which may be useful for designing interventions in the sense of improve this practice.
