existam cerca de 200 milhões de pessoas infetadas em todo o mundo. O aparecimento de
cerca de 3 a 4 milhões de novos casos por ano conduz ao aumento da frequência de casos
de cirrose hepática e de carcinoma hepatocelular. Esta doença é causada pela infeção do
indivíduo com o vírus da hepatite C, sendo a via de transmissão principal a via parenteral.
Apesar dos indivíduos infetados poderem sentir fadiga, a hepatite C evolui de forma
assintomática e, por isso, é crucial a implementação de medidas preventivas de forma a
contornar os fatores de risco associados à transmissão da infeção. Os primeiros fármacos
aprovados para o tratamento desta infeção são imunomoduladores e apenas garantem
eficácia em 50-80% dos doentes tratados. Recentemente, a introdução de fármacos de ação
direta permitiu um aumento significativo da resposta à terapêutica. Contudo, a eficácia da
terapêutica é condicionada por fatores virais e do hospedeiro e, assim, a sua identificação é
crucial na determinação da terapia a instituir. Tendo em consideração a estrutura viral, o
ciclo de vida do vírus e os fatores que influenciam a resposta à terapêutica, o
desenvolvimento contínuo dos fármacos não só é decisivo no combate à evolução da doença
mas também na melhoria dos efeitos indesejáveis dos fármacos disponíveis. Hepatitis C is a viral disease characterized by the inflammation of the liver. It is estimated that approximately 200 million people are infected worldwide. The appearance of about 3 to 4 million new cases each year leads to an increased frequency of cases of liver cirrhosis and hepatocellular carcinoma. This disease is caused by infection of the individual with the Hepatitis C virus, being the main transmission via the parenteral route. Despite the affected people may feel some fatigue, hepatitis C evolves asymptomatically and therefore is crucial to implement preventive measures in order to circumvent the risk factors associated with the transmission of this disease. The first drugs approved for treatment of disease are immunomodulators, which only guarantee effectiveness in 50-60% of treated patients. But recently, the introduction of direct action drugs has allowed a significant increase of therapy response. However, the success of the therapy is conditioned by viral and host factors, and thus, their identification is the decisive factor in the choosing of the most suitable therapy for each person. Considering the viral structure, the virus life cycle and the factors that influence the therapeutic efficacy, it is critical to continue developing drugs that fight the disease progression but that also improve the undesirable effects of available drugs. |
