Title: Características da anemia em doentes idosos internados

Abstract: Introdução: A anemia é uma das patologias mais diagnosticadas em todo o Mundo, constituindo um factor que contribui para a morbilidade geral. Os principais objectivos deste trabalho foram: avaliar a frequência desta patologia em determinada população, identificar o tipo de anemia e causa mais frequente, avaliando a estratégia terapêutica instituída. Identificar a existência de uma relação significativa entre a prevalência de anemia e de outras patologias frequentes na população considerada. Métodos: Foi feita a recolha de dados dos Processos Clínicos de alguns doentes internados na Enfermaria do Serviço de Medicina Interna dos Hospitais da Universidade de Coimbra, no ano de 2009. A selecção da amostra de 252 doentes foi feita de acordo com critérios de anemia previamente definidos. Procedeu-se ao tratamento estatístico dos dados recolhidos. Resultados: Durante o período de tempo considerado, 252 doentes (25% da amostra inicial) foram diagnosticados com anemia. Destes, 34,9% apresentavam idades entre os 81 e os 90 anos, sendo que 61,5% tinham entre os 71 e os 90 anos. O tipo de anemia mais identificado foi a Anemia Microcítica Hipocrómica (52%). Em 94,8% dos doentes, não foi possível identificar a causa subjacente. As causas mais identificadas foram: Deficiência de Ferro (2,0%) e Doença Crónica (1,2%). Apenas 47,2% foram submetidos a tratamento específico. A terapêutica mais utilizada isoladamente ou em associação, consistiu na administração de um composto de Ferro (25,8%). Discussão: A frequência de anemia nestes doentes aproxima-se dos resultados encontrados na população geral e em Unidades de Internamento de Curta Duração. As faixas etárias mais identificadas, neste estudo, como apresentando maior prevalência de anemia, aproximam-se do esperado e mencionado na literatura disponível. O tipo de anemia mais identificado nestes doentes, correspondeu à Anemia Microcítica Hipocrómica. Também este resultado vai de encontro ao esperado. Apenas numa pequena minoria (5,2%) foi possível a identificação da causa subjacente, correspondendo a um valor inferior ao previsto. As duas causas mais frequentes foram a Deficiência de Ferro (2,0%), e a Doença Crónica (1,2%). As patologias mais comuns nos doentes em estudo corresponderam a Hipertensão Arterial (32,1%), Patologia neurológica (30,1%) e Patologia Cardíaca (26,6%). Identificou-se uma diferença estatisticamente significativa entre a prevalência da Hipertensão Arterial, Diabetes Mellitus, Patologia Neurológica e Asma, na Amostra considerada, relativamente à encontrada na População Geral. Este resultado necessita de estudos posteriores, para que conclusões mais detalhadas possam ser retiradas. Conclusão: Os objectivos inicialmente propostos, embora com as limitações próprias de um estudo retrospectivo foram respondidos nos resultados obtidos. No geral verificou-se que a anemia não foi muito valorizada durante o internamento destes doentes, possivelmente por cursar com patologias agudas mais graves. Por fim, haverá necessidade de dar mais atenção a esta situação clínica considerando que pode, quando tratada, melhorar o prognóstico em muitos casos
Introduction: Anemia is the most diagnosed pathologies throughout the world, a factor which contributes to morbidity. The main aims of this paper were to evaluate the frequency of this pathology in a given population, identify the type of anemia and its most frequent cause, and evaluate the established therapeutic strategy. Also, it aims to identify the existence of a significant relationship between the prevalence of anemia and other frequent pathologies within a given population. Methods: A collection of data of the clinical files of some admitted patients was carried out at the Internal Medicine Ward of the University Of Coimbra Hospitals in the year 2009. The selection of the sample of 252 patients was carried out in accordance with the criteria of pre-defined anemia. Statistical treatment of the information gathered was then performed. Results: During the period of time analyzed, 252 patients (25% of the initial sample) were diagnosed with anemia. From these, 34, 9% were between 81 and 90 years of age, while 61, 5% were aged between 71 and 90. The most identified type of anemia was Hypochromic Microcytic Anemia (52%). It was not possible to identify the underlying cause in 94, 8% of the patients. The most identified causes were Iron Deficiency (20%) and Chronic Illness (1, 2%). Only 47, 2% underwent a specific treatment. The most commonly used therapeutic, administered alone or in association, was a compound of Iron (25, 8%). Discussion: The frequency of anemia in these patients is approximate to the results found in the general population in Short Stay Inpatient Units. In this study, the age range which presented the highest prevalence of anemia approximates the expected and those mentioned in the available literature. The most identified type of anemia in these patients corresponded to Hypochromic Microcytic Anemia. This result also meets the expected. It was only possible to identify a small minority (5, 2%) of underlying cause, which corresponds to a value inferior to the expected. The two most common causes were Iron Deficiency (2, 0%) and Chronic Illness (1, 19%). The most common pathologies of the patients analyzed corresponded to Hypertension (32, 1%), Neuropathology (30, 1%) and Cardiac Pathology (26, 64%). A significant statistical difference was identified between the prevalence of Hypertension, Diabetes Mellitus, Neuropathology and Asthma in the sample considered and those found in the general population. This result needs further studies so that more detailed conclusions can be reached. Conclusion: The objectives originally proposed, despite the limitations of a retrospective study, were answered within possibilities. In general, it was found that the anemia was not highly valued during hospitalization of patients, possibly due to the presence of acute conditions more severe. Finally, it is recommended that more attention should be paid to this clinical situation considering that it can, when treated, improve the prognosis in many cases.
