Title: Agressões sexuais : a importância do perito em medicina legal no serviço de urgência

Abstract: Introdução: Permanece incógnito o número de casos de agressões sexuais uma vez que apenas uma minoria dos casos é denunciada devido a tabus sociais e culturais, a valores pessoais ou negação. Apesar das discrepâncias estatísticas de país para país os seus valores são sempre assustadores e, segundo dados do Departamento de Justiça dos Estados Unidos da América, 75% das vítimas necessitam de cuidados médicos após a agressão. Assim, aos médicos do Serviço de Urgência cabe muitas vezes a abordagem inicial das vítimas, devendo ser providenciado um local reservado, simultaneamente dispondo de tempo e conhecimentos práticos forenses que favoreçam uma abordagem sistemática de avaliação e tratamento. Objectivos: O presente trabalho visa inferir sobre a capacidade de resposta dos Médicos que prestam Serviço de Urgência nos Hospitais da Universidade de Coimbra na avaliação, recolha de amostras forenses e tratamento das vítimas de agressão sexual. Metodologia: Realização e análise de inquéritos anónimos aos médicos das especialidades de Cirurgia e Ginecologia, uma vez que, no Hospital considerado, para eles são triadas as vítimas de agressões sexuais. Os dados foram tratados usando o SPSS versão 16.0. Resultados: 57,1% dos inquiridos responderam ao inquérito (mais Ginecologistas que Cirurgiões, e mais internos que especialistas). Nenhum respondente acertou menos de 11 perguntas e nenhum interno de Ginecologia menos que 16. A maioria dos respondentes acertou entre 16 a 20 questões. Apenas Internos acertaram mais do que 20 perguntas (3 internos de Cirurgia e 1 de Ginecologia). As questões providas de maiores dificuldades foram as relacionadas com a correcta técnica de colheita de fluídos biológicos, prazo geralmente aconselhado para realização da mesma e seu envio para análise laboratorial. Conclusões: Perante médicos que todos os dias se encontram na iminência de terem ao seu cuidado vítimas de agressão sexual, é importante investir numa maior formação forense, evitando a revitimização que um exame inadequado ou a espera pelo Perito Médico-Legal de chamada causaria. É fundamental a criação de condições para o atendimento destas vítimas no Serviço de Urgência, providenciando-se o material necessário.
Introduction: It remains unknown the number of sexual assaults since only a minority is reported to the authorities. This is mostly the result of a series of social and cultural taboos, personal values or denial. All over the world the percentage of sexual aggressions are rising to outstanding numbers, and data from the United States of America Department of Justice say that nearly 75 % of the victims will require medical care after the assault. This means that, in the great majority of cases, the first approach to the victim is made by the doctors that are attending the Emergency Room, and therefore it must be provided a confidential space and the time and material necessary for a proper evaluation and management of the victim. The conductor of the evaluation must be provided with the needed practical forensic knowledge for a correct and systematic approach. Objectives: The purpose of this work is to infer about the response given by Surgeons and Gynecologists of Hospitais da Universidade de Coimbra in the primary evaluation and treatment of sexually assaulted victims. Methods: While carrying out this work, a questionnaire was anonymously made, to Surgeons and Gynecologists, working at the hospital in question, since sexual assault victims are initially screened for these kind of physicians. All the data collected was then processed using SPSS version 16.0. Results: 51,7% of all doctors inquired answered the questionnaire (Gynecologist more than Surgeons and residents more than specialists). The majority answered 16 to 20 correct answers. No doctor had less than 11 correct answers. Only residents had more than 20 answers correct (3 Surgeons and 1 Gynecologist. The questions provided with greater difficulties where the ones related with the correct technique of collecting samples and of sending them to analyze. The period during which the samples must be collected was also a source of confusion. Agressões sexuais – A importância do Perito em Medicina Legal no Conclusions: It is important to implement measures to enlarge forensic knowledge, so that physicians that often find themselves observing and treating sexual assault victims may do it properly, thus preventing the re-victimization that an inadequate exam or the painful waiting for a Coroner would cause. It is also essential to provide the right material and the necessary conditions for the execution of a sexual exam while in the Emergency Department.
