Title: Estratégias terapêuticas antitrombóticas na fibrilhação auricular e doença cerebrovascular isquémica

Abstract: Introdução e objectivos: A fibrilhação auricular (FA) é a arritmia mais frequente e é a principal causa de doença cerebrovascular isquémica (DCVI). A anticoagulação oral é o tratamento preconizado para doentes de alto risco cardioembólico, mas as limitações inerentes ao uso de antagonistas da vitamina K condicionam a sua prescrição adequada. Neste trabalho pretendemos avaliar o perfil epidemiológico das DCVI agudas, o estado diagnóstico e terapêutico da FA e o resultado funcional da DCVI numa população hospitalar. Métodos: Avaliámos prospectivamente 458 casos consecutivos de DCVI admitidos na Unidade de Acidentes Vasculares Cerebrais e enfermaria cerebrovascular de um hospital central. Para cada caso identificámos a síndrome clínica, etiologia, gravidade e resultado funcional. Nos casos de cardioembolia registámos a causa subjacente, altura de diagnóstico de FA quando presente, tratamento anticoagulante prévio, perfil de coagulação à entrada e anticoagulação à alta. Comparámos gravidade, resultado funcional e mortalidade entre as etiologias e procurámos associações com variáveis demográficas. Resultados: A cardioembolia é a principal causa de DCVI (33,4% dos eventos), sendo a FA responsável por 91,5% dos casos. Em 71,2% dos casos o diagnóstico de FA antecedeu a DCVI; 34,5% destes doentes recebiam terapêutica anticoagulante. Encontrámos diferenças significativas na gravidade, resultado funcional e mortalidade entre as várias etiologias, sendo a cardioembólica a mais grave. Detectámos maior prevalência da cardioembolia nos idosos e no sexo feminino. Conclusões: Apesar da FA ser adequadamente diagnosticada, observámos uma subutilização preocupante da terapêutica anticoagulante, colocando os portadores da arritmia em risco de sofrerem DCVI cardioembólica com elevada gravidade e mortalidade.
Introduction and objectives: Atrial fibrillation (AF) is the most common arrhythmia and the leading cause of ischemic stroke. Oral anticoagulation therapy (OAT) is the recommended treatment for patients with high cardioembolic risk, but underlying restrictions to the use of vitamin K antagonists limit their appropriate prescription. The purpose of our study was to assess the epidemiologic profile of acute ischemic stroke, diagnostic and therapeutic state of AF and functional outcome of stroke in a hospital-based population. Methods: We prospectively analyzed 458 consecutive cases of acute ischemic cerebrovascular disease that were admitted to the Stroke Unit and cerebrovascular ward of a central hospital. For each case we identified clinical syndrome, etiology, severity and functional outcome. In case of cardioembolism we registered the underlying cause, occasion of AF diagnosis if present, previous OAT, coagulation profile at admission and OAT at discharge. We compared severity, functional outcome and mortality between different etiologies and looked for associations with demographic variables. Results: Cardioembolism is the leading cause of stroke (33.4% of all events), with AF being responsible for 91.5% of those cases. In 71.2% diagnosis of AF preceded stroke; 34.5% of those patients were receiving OAT. We found significant differences in severity, functional outcome and mortality between etiologies, being cardioembolism the most serious one. We detected higher prevalence of cardioembolism in the eldery and in women. Conclusions: Despite being adequately diagnosed, we found a preoccupying underprescription of OAT in AF, putting patients with the arrhythmia in risk of suffering potentially severe and lethal cardioembolic strokes
