dc.description.abstractIntrodução: Tem havido um reconhecimento crescente de que a Doença de Parkinson (DP) constitui não só um distúrbio motor, mas também uma doença sistémica com múltiplas manifestações não motoras. Uma das mais relevantes constitui o défice olfactivo que parece ocorrer precocemente e ao longo de toda a progressão da doença. A avaliação olfactiva afigura-se como um ferramenta particularmente útil no diagnótico precoce e diferencial da DP. Objectivo: Caracterizar a função olfactiva numa amostra de pacientes com DP e avaliar a relação da mesma com determinantes clínicos e epidemiológicos. Design do estudo: Estudo prospectivo analítico observacional tipo caso-controlo. Doentes e métodos: Foram avaliados clinicamente 45 indivíduos com DP e 30 indivíduos normais através da avaliação neurológica e do protocolo de olfacto (teste do limiar olfactivo e supraliminar). Resultados: Os limiares olfactivos dos indivíduos com DP foram significativamente superiores aos dos controlos (p<0,05). A capacidade de identificação olfactiva dos doentes parkinsónicos foi inferior à dos controlos (p<0,05). As alterações observadas foram independentes da sintomatologia motora, não motora e do número de sintomas actuais. Conclusões: A disfunção olfactiva constitui um fenómeno muito prevalente na DP. A sua avaliação revela-se especialmente pertinente não só no diagnóstico precoce, mas também na identificação e orientação de pacientes com maior compromisso da sua segurança e qualidade de vida.por
dc.description.abstractIntroduction: It has become increasingly apparent that Parkinson’s disease (PD) is no longer a purely motor disease, but rather a systemic disease with numerous nonmotor symptoms. One of the most notable among such is the decrease in the sense of smell that appears to occur in the early stages of the disease and throughout its progression. The olfactory testing appears to have a wide application in the early recognition and differential diagnosis of PD. Objective: The purpose of this study was to evaluate the olfactory function in PD patients and to verify its relation with several clinical and epidemiologic items. Study design: Prospective case-control study. Patients and Methods: 45 PD patients and 30 healthy controls were tested for their neurologic performance and olfactory function, concerning both detection and identification tests. Results: PD patients revealed a significantly higher threshold in the olfactory detection test (p<0,05). The score in the identification test for PD patients was lower than for controls (p<0,05). No association was found between the olfactory dysfunction and the number of present symptoms or the presence of motor or nonmotor signs. Conclusion: The prevalence of olfactory dysfunction in PD patients tends to be very high. It affects the detection and identification of odorants. Its evaluation is very important to early diagnose and closely follow PD patients, not only to improve their quality of life, but also to provide them a saffer environment.por
