dc.description.abstractIntroduction: Liver resection combined with neoadjuvant chemotherapy (NAC) has reported notable results in patients with colorectal liver metastases (CRLM). Tumoral response to NAC is associated with specific histopathologic patterns with prognostic implications. The primary objective of this study was to evaluate the influence of pathological findings on overall survival (OS), disease-free survival (DFS) and liver recurrence-free survival (LRFS) and secondarily to analyze the role of NAC and clinical features on patients’ outcome. Patients and Methods: Analysis of clinical and outcome data from 110 patients who underwent first CRLM resection between January 2010 and July 2013 was carried out. Blinded pathological review of histological material of several parameters: resection margin, tumor regression grade (TRG), tumor thickness at the tumor-normal interface (TTNI) and the growth pattern (GP). Results: The median survival following hepatic resection was 52 months and 3- and 5- year Kaplan-Meier estimates were 69 and 48%, respectively. Seventy-four patients developed recurrent disease. Oxaliplatin-based chemotherapy was significantly associated with a pushing GP. A positive resection margin was an independent predictor of decreased DFS (p=0.018) and LRFS was strongly reduced by the absence of histologic tumor response (p=0.018). The pushing pattern had an adverse impact on both OS (p=0.007) and DFS (p=0.004) on multivariate analysis. Conclusion: The prognostic value of histopathological features in patients who underwent CRLM’s resection is undeniable. The pushing GP was related with worse prognosis. Further studies are required to clarify the biological mechanisms underlying these findings in order to enhance a more personalized and efficient treatment for these patientspor
dc.description.abstractIntrodução: A resseção hepática em associação com a quimioterapia neoadjuvante (QNA) tem tido resultados notáveis em doentes com metástases hepáticas de carcinoma colorectal (MHCCR). A resposta tumoral à QNA está associada a padrões histopatológicos específicos com implicações prognósticas. Este estudo teve como objetivo primário avaliar a influência das carcterísticas histopatológicas na sobrevivência global (SG), sobrevivência livre de doença (SLD) e sobrevivência livre de recidiva hepática (SLRH). Secundariamente, pretendeu-se analisar o papel da quimioterapia neoadjuvante e dos parâmetros clínicos no prognóstico dos doentes. População e métodos: Analisaram-se os dados clínicos de 110 doentes que foram submetidos a primeira hepatectomia por MHCCR entre janeiro de 2010 e julho de 2013. Foi realizada uma revisão com ocultação do material histológico em particular quanto a: margem de resseção cirúrgica, grau de regressão tumoral, espessura tumoral na interface tumor-parênquima e padrão de crescimento tumoral. Resultados: A sobrevivência mediana após hepatectomia foi de 52 meses e de 69 e 48% aos 3 e 5 anos, respectivamente. Setenta e quatro doentes desenvolveram recidiva. Os doentes sujeitos a regimes de quimioterapia com oxaliplatina tiveram uma maior incidência do padrão de crescimento tumoral expansivo. Uma margem cirúrgica positiva demonstrou ser um fator independente de pior SLD (p=0.018) e a SLRH foi substancialmente reduzida em doentes com ausência de resposta histológica tumoral (p=0.018). O padrão de crescimento expansivo afetou de forma negativa tanto a SG (P=0.007) como a SLD (p=0.004) numa análise multivariada. Conclusão: O valor prognóstico dos parâmetros histopatológicos em doentes submetidos a resseção hepática por MHCCR é inegável. O padrão de crescimento tumoral expansivo foi associado a um pior prognóstico. São necessários mais estudos para clarificar os mecanismos biológicos subjacentes a estes achados de forma a providenciar protocolos terapêuticos mais personalizados e eficientes.por
