dc.description.abstract | Anemia is a condition with high prevalence in the elderly population, however, because of its clinical range it becomes very complicated and controversial the existence of a standardized diagnosis and therapeutic model, making the analysis of the relevant scientific evidence very problematic. Anemia is strongly associated with increased risk of morbidity and mortality, particularly regarding the loss of functionality, the increased risk of cardiovascular and neurological disease and decreased quality of life. Anemia is strongly related with age and given the current demographic changes with the progressive aging of the population, this medical situation will be a problem with large repercussions in the future. Etiologically anemia in the elderly is mainly caused by nutritional deficits, by the simultaneous chronic diseases and it also can have an unknown origin. The most effective treatment include the correction of the deficits with supplements of iron, vitamins or folates. It is also important to focus on managing the underlying disease such as intestinal bleeding. Inflammatory anemia is associated with infectious, inflammatory and neoplastic diseases. Their pathophysiological basis is linked to iron deficiency induced by hepcidin and to the suppression of erythropoiesis by inflammatory mechanisms. In these patients is essential to identify the deficits and associated diseases in order to achieve an effective intervention. dc.description.abstract | A anemia é uma condição com alta prevalência na população geriátrica, no entanto, a sua abrangência clínica torna muito complexa, e até controversa, a existência de um modelo diagnóstico e terapêutico padronizado, dificultando a análise da correspondente evidência científica. A anemia está fortemente associada a um risco aumentado de morbilidade e mortalidade, nomeadamente no que concerne à perda de funcionalidade, ao risco aumentado de doença cardiovascular e neurológica e à diminuição de qualidade de vida. Sendo a prevalência desta condição fortemente relacionada com a idade, as atuais alterações demográficas com o progressivo envelhecimento da população, fazem prever que a anemia será um problema clínico com grande representação no futuro. As grandes etiologias de anemia em geriatria envolvem os défices nutricionais, a concomitância de doença crónica e a origem desconhecida. A terapêutica mais eficaz consiste no suporte dos défices de substratos à eritropoiese, nomeadamente com suplementos de ferro, folatos ou vitaminas. Concomitantemente, será fundamental o tratamento da causa subjacente, por exemplo em casos de hemorragia intestinal. A anemia da doença crónica ou inflamatória está associada a patologias infeciosas, inflamatórias e neoplásicas. A sua base fisiopatológica está relacionada com o défice de ferro induzido pela hepcidina e pela supressão inflamatória da eritropoiese. Nestes doentes é fundamental identificar os défices e doenças associadas de modo a intervir eficazmente.
