Title: Custo-efetividade da utilização de um teste farmacogenético para o Alopurinol em doentes portugueses diagnosticados com gota

Abstract: Introdução: Nos últimos anos, fruto dos recentes avanços realizados ao nível da farmacogenética, foi identificado em indivíduos portadores do alelo HLA-B*5801 um risco acrescido de ocorrência de reações de Síndrome de Stevens-Johnson/Necrólise Epidérmica Tóxica (SSJ/NET) quando tratados com alopurinol. Tendo em conta o papel fundamental do alopurinol no tratamento hipouricemiante de doentes diagnosticados com Gota, vários autores têm proposto a utilização de um teste farmacogenético para este medicamento. Objetivos: Avaliar a relação custo-efetividade da utilização de um teste farmacogenético para o alopurinol em doentes portugueses de origem caucasiana diagnosticados com Gota, face à sua não utilização. Complementarmente, pretendeu-se estimar um custo máximo de comercialização para o teste e avaliar o impacto da sua introdução generalizada sobre o orçamento anual do SNS. Métodos: Todas as sequências de eventos, relacionadas com as alternativas em análise, foram estruturadas num modelo analítico sob a forma de uma árvore de decisão, para um horizonte temporal de três meses. Este estudo foi realizado para a perspetiva dos pagadores e para a perspetiva do Serviço Nacional de Saúde (SNS), considerada como entidade financiadora. Os dados de efetividade foram estimados a partir de estudos publicados e sempre que possível foram utilizados dados relativos à população portuguesa. As relações de custo-efetividade para cada alternativa foram comparadas através do cálculo da razão incremental de custo-efetividade (RICE), cujo valor traduz o custo adicional por cada reação de SSJ/NET evitada. A incerteza associada aos parâmetros utilizados foi avaliada através de análise de sensibilidade univariada. Resultados: A utilização do teste foi associada a uma diminuição de 0,00125 reações de SSJ/NET por cada doente tratado com alopurinol comparativamente à alternativa base. Foram estimados custos diretos de tratamento de 11,50€ e 53,84€ para a perspetiva dos pagadores e de 9,55€ e 51,81€ para a perspetiva do SNS. A conjugação destes resultados permitiu calcular uma razão incremental de custo-efetividade de 33.872€/reação evitada para a perspetiva dos pagadores e de 33.808€/reação evitada para a perspetiva do SNS. Considerando que o SNS poderá estar disposto a financiar o teste até ao ponto em que o custo suportado pela intervenção seja igual ao do tratamento de uma reação de NET foi 10 estimado um preço máximo de comercialização para o teste de 8,82€. Ao preço unitário de 45,30€, a implementação generalizada do teste seria responsável por um impacto sobre o orçamento anual do SNS de +419.243,04€. Considerando o valor do preço máximo estimado, o impacto orçamental foi avaliado em apenas +55.792,80€. Conclusões: De acordo com os valores de custo incremental obtidos para ambas as perspetivas, não será de prever que, ao preço atual de 45,30€, o SNS tome uma decisão no sentido de considerar a utilização do teste como uma opção custo-efetiva. Para além disso, a implementação generalizada desta tecnologia seria responsável por uma alocação significativa de recursos do SNS. No entanto, admite-se que a utilização do teste poderá ser considerada pelo SNS como uma alternativa vantajosa sempre que os custos a financiar por cada intervenção não ultrapassem o valor máximo de 8,82€. A realização deste trabalho permitiu, ainda, identificar importantes lacunas de informação, principalmente relacionadas com a escassez de dados clínicos referentes à incidência das reações de SSJ/NET em doentes tratados com alopurinol e com o impacto deste tipo de reações adversas sobre a qualidade de vida dos doentes.
Introduction: In recent years, resulting from recent advances performed at the level of phamacogenetics, it was possible to demonstrate that for individuals carrying the allele HLA-B*5801 there is an increased risk of occurrence of Stevens-Johnson Syndrome/Toxic Epidermal Necrolysis reactions (SJS/TEN) when treated with allopurinol. Taking into account the role of allopurinol in hyperuricemic treatment of patients diagnosed with Gout, some authors have proposed the use of a phamarcogenetic test for this drug. Objectives: Evaluate the cost-effectiveness relationship for the use of a phamarcogenetic test for allopurinol in portuguese patients of caucasian origin diagnosed with Gout, face to its non-use. Complementarily, to estimate the impact of the widespread introduction of the test on the annual budget of the SNS. Methods: All sequences of events, related to the alternatives under analysis, were structured in an analytical model in the form of a decision tree, adopting a three-month time horizon. This study was conducted for the payers perspective and for the National Health Care Service perspective, regarded as a funding body. The data of effectiveness were estimated from published studies and whenever possible were used data from Portuguese population. The cost effectiviness relationships for each alternative where compared by calculating the incremental cost-effectiveness ratio (ICER), whose value reflected the additional cost for each reaction of SSJ/NET avoided. The uncertainty associated with the parameters used was measured by univariate sensitivity analysis. Results: The use of the test was associated with a decrease of 0,00125 SJS/TEN reactions for each new patient treated with allopurinol compared to the alternative basis. Were estimated direct costs of treatment of 11,50€ and 53,84€ for the payers perspective and 9,55€ and 51,81€ for the National Health Care Service perspective. The combination of these results allowed to calculate an incremental cost effectiveness ratio of 33,872€/avoided reaction for the payers perspective and 33,808€/avoided reaction for the National Health Care Service perspective. To the unit price of 45,30€, the widespread implementation of the test would be responsible for an impact on the National Health Care Service annual budget of +419,243,04€. Considering the value of the maximum price estimate, the budgetary impact was rated at only +55,792,80€. 12 Discussion/Conclusions: According to the obtained values of incremental cost for both perspectives, it´s not predictable that, at a unit price of 45,30€, the takes a decision in order to consider the use of the test as a cost-effective option. In addition, the widespread implementation of this technology would be responsible for a significant allocation of National Health Care Service resources. However, it is assumed that the use of the test could be considered by the National Health Care Service as an advantageous alternative whenever the costs to be financed for each intervention do not exceed the maximum value of 8,82€. This work allowed, also, to identify important information gaps, mainly related to the lack of clinical data regarding the incidence of SJS/TEN reactions in patients treated with allopurinol, and the impact of such adverse reactions on patient’s quality of life.
