As funções executivas em crianças e adolescentes com Perturbação de Hiperatividade e Défice de Atenção: revisão da literatura

Abstract

A presente revisão da literatura foi realizada com o intuito de verificar em que medida os défices nas funções executivas estão relacionados com a Perturbação de Hiperatividade e Défice de Atenção (PHDA). Para tal, foram consultados estudos que avaliaram estas funções em crianças e adolescentes com PHDA com recurso a baterias de instrumentos e questionários comportamentais. Os resultados observados em estudos realizados com baterias de instrumentos apresentam algumas inconsistências, no entanto existe algum consenso no que concerne a dificuldades na velocidade de processamento, na inibição e na flexibilidade cognitiva. A maior parte dos estudos não encontrou evidências suficientes que permitissem fazer uma distinção dos subtipos de PHDA. Nos estudos com questionários existiu mais consenso relativamente à existência de défices nas FE, com a maior parte dos estudos a encontrarem evidências claras da distinção entre crianças e adolescentes com e sem PHDA. Alguns estudos conseguiram, da mesma forma, encontrar provas da distinção entre os subtipos combinado e desatento da perturbação. Tendo em conta estes achados julga-se necessária a continuação da investigação destas funções nesta população, com métodos de seleção e amostragem mais homogéneos (e.g., comorbilidades), visando a utilização de uma mesma bateria de instrumentos entre os estudos de forma à obtenção de resultados mais uniformes e comparáveis entre si, permitindo, desta forma, uma melhor visão sobre a existência ou ausência de défices nas FE.

This literature review intended to verify in what extent way deficits in executive functions are associated with Attention Deficit Hyperactivity Disorder. In order to do so, studies have been analyzed that assessed these functions in children and adolescents with the disorder using performancebased measures as well as parents and teachers’ behavioral ratings. The results observed in studies that used performance-based measures present some inconsistencies, nevertheless, consensus was obtained regarding dificulties in processing speed, inibition and cognitive flexibility or setshifting. The majority of the studies was not able to find evidences that allowed to differenciate amonsgt the ADHD subtypes. On the other hand, studies that used parents and teachers’ behavioral ratings attained more coherent results concerning the existence of deficits in executive funtions in children and adolescents, being able to distinguish amid children and adolescents with ADHD, who achieved much higher results, and controls. Some of these studies were also able to find evidences that permitted to distinguish between the combined and inattentive subtypes of this disorder. Having this finding in mind, it is of utmost importance to keep studying and investigating these functions in children and adolescents with this disorder, aiming to homogenize methods of selection and sample features (e.g., comorbidities) and using the same performance-based measures across studies in order to allow a better comparision of the results and a clearer vison of the presence or absence of executive function deficits amongst this population.
