Title: A influência da sitagliptina no vasorrelaxamento induzido pela acetilcolina na aorta isolada de rato: comparação com um modelo animal de Diabetes Mellitus tipo 1 Maria

Abstract: As Gliptinas são fármacos inibidores da dipeptidil-peptidase IV (DPP-IV), que constituem uma nova classe de agentes orais para o tratamento da DMT2 e a sitagliptina é conhecida pelo seu interesse terapêutico através do aumento dos níveis de hormonas incretinas activas GLP-1 e GIP. A DPP-IV degrada rapidamente o GLP-1 (7-36), obtendo-se o GLP-1 (9-36), que inicialmente se pensava ser biologicamente inactivo. Posteriormente, o GLP-1 (9-36) mostrou efeitos independentes do receptor GLP-1R na melhoria da função cardíaca e vasodilatação. Os estudos também sugerem que os efeitos cardioprotectores de ambos o GLP-1 e o GLP-1 (9-36) são parcialmente atribuídos ao seu mecanismo vasodilatador que é dependente de óxido nítrico (NO)/cGMP e também ao aumento da estabilidade do SDF-1α, na medida em que prolonga o seu tempo de vida e a sua acção fisiológica, envolvendo o recrutamento de células progenitoras endoteliais (EPCs) a partir da medula óssea. As EPCs contribuem para a endotelialização dos vasos lesados e a neovascularização induzida por eventos isquémicos. Um dos objectivos do presente estudo consistiu em avaliar o efeito da sitagliptina na reversão/atenuação da disfunção endotelial, por um mecanismo independente do aumento de secreção de insulina, num modelo animal de diabetes do tipo 1, isto é, diabetes induzida por STZ em ratos Wistar. A diabetes foi induzida com uma injecção intraperitoneal única de STZ (65 mg / kg em 10 mM de tampão citrato, pH 4,5) a ratos Wistar machos (8 semanas de idade). Após 2 semanas de indução do diabetes, os animais foram divididos em quatro grupos: controlos, diabéticos e controlos e diabéticos tratados com sitagliptina 5 mg/kg/dia durante as duas semanas seguintes. Os ratos foram em seguida anestesiados (cocktail intraperitoneal 2 mg/kg de uma solução 2:1 (v/v) de 50mg/mL de cloridrato de cetamina em 2,5% clorpromazina) e as amostras de sangue colhidas a partir da veia jugular para a medição de glicose, insulina e níveis de hemoglobina glicosilada (HbA1c) e medição da actividade da DPP-IV. Os animais foram então sacrificados e três animais de cada grupo foram utilizados para o isolamento da aorta. Após uma pré-contracção com 10μM NA, procedeu-se ao registo do relaxamento isométrico de duas curvas cumulativas concentração-resposta (CR) de ACh (ACh, 0,01-90 μM), na ausência e na presença do éster metílico de NG-Nitro-L-arginina (NG-Nitro-L-arginine methylesther, L-NAME), um inibidor da eNOS, adicionado ao banho de órgãos 30 minutos antes da segunda curva CR, que permitiu confirmar a produção de NO induzida por ACh em todos os grupos. Cada anel funcionou como controlo de si próprio, já que as respostas à ACh da segunda curva foram expressas em percentagem do relaxamento máximo obtido na primeiracurva CR do respectivo anel. Os resultados finais de cada experiência foram expressos em percentagem da resposta obtida nos anéis controlo. A ACh foi mais potente em induzir o vasorrelaxamento em ratos de controlo tratados com sitagliptina (pEC50: 6,77 ± 0,09, n = 12, p= 0,039 vs. ratos controlo não tratados pEC50: 6,41 ± 0,14, n = 12, p= 0,039). Inesperadamente, em ratos diabéticos tratados, a sitagliptina parece comprometer o efeito vasorrelaxante mediado pela ACh com uma redução significativa da percentagem de inibição da contracção induzida pela NA (Emáx: 27,1 ± 5,1%, n = 12 vs. ratos diabéticos não tratados Emáx de 59,5 ± 8,2%, n = 12, p = 0,003). Para estudar os mecanismos subjacentes aos resultados obtidos anteriormente, recorreu-se à técnica de Western blotting para observar a expressão do GLP-1R e das formas totais e fosforiladas das enzimas eNOS e Akt, de forma a avaliar o estado da via de sinalização Akt/eNOS, na presença e na ausência de tratamento com sitagliptina, em ratos controlo e ratos diabéticos. Nos ratos diabéticos observou-se um aumento significativo na expressão do GLP-1R comparativamente com os animais controlo. No grupo de ratos diabéticos sujeitos a tratamento com sitagliptina, parece haver uma tendência para aproximação da situação controlo com uma redução da expressão do GLP-1R. No que diz respeito aos níveis de expressão de Akt activa, determinados através da razão p-Akt/Akt total, não foram observadas diferenças estatisticamente significativas entre os grupos estudados. No entanto, a administração de sitagliptina tendencialmente aumentou a razão p-Akt/Akt total em ratos controlo e ratos diabéticos, embora nestes últimos o aumento tenha sido mais ténue. Por fim, não foram observadas diferenças estatisticamente significativas nos níveis de expressão de eNOS activa entre os quatro grupos em estudo, determinados através da razão p-eNOS/eNOS total. Recorrendo a marcadores de stress oxidativo, avaliou-se a presença de aniões superóxido nos anéis de aorta dos 4 grupos de animais. Verificou-se um aumento no stress oxidativo nos anéis de aorta de ratos diabéticos, traduzido por um aumento da intensidade da fluorescência, comparativamente aos respectivos animais controlo. A administração de sitagliptina diminuiu significativamente a intensidade da fluorescência, quer nos animais controlo, quer nos diabéticos. Por fim, no sentido de complementar os estudos funcionais, realizaram-se estudos histoquímicos de modo a caracterizar a micro-anatomia dos anéis de aorta dos 4 grupos de animais. As alterações histológicas observadas no presente estudo demonstraram que a organização estrutural do tecido aórtico foi perturbada em ratos com diabetes induzida por STZ, tendo a administração de sitagliptina nestes animais conduzido a um aumento da densidade da matriz numa tentativa de aproximação da situação controlo.Em suma, os resultados obtidos permitem inferir que, na presença de diabetes, pode ocorrer um comprometimento da via de sinalização PI3K/Akt/eNOS, conduzindo ao desenvolvimento de disfunção endotelial diabética, caracterizada maioritariamente por anomalias na vasodilatação mediada pelo NO, associadas ao stress oxidativo. De facto, os efeitos benéficos da sitagliptina na função vascular, observados no grupo controlo deste estudo e que são independentes da secreção de insulina, não foram observados na presença da patologia, podendo estar subjacentes fenómenos que comprometem a via de sinalização comum e que não são revertidos pelo fármaco, pelo menos ao fim do curto período de tratamento a que os animais foram sujeitos.
Gliptins are inhibitors of dipeptidyl peptidase (DPP-IV) that constitute a novel class of oral agents for type 2 diabetes treatment and sitagliptin is known to have therapeutic interest through increased levels of active incretin hormones GLP-1 (glucagon-like peptide 1) and GIP (glucose-dependent insulinotropic peptide). DPP-IV rapidly degrades GLP-1 (7-36) yielding GLP-1 (9-36), which was initially believed to be biologically inactive. Latter on GLP-1 (9-36) showed independent effects of GLP-1R receptor to improve cardiac function and vasodilation. Studies also suggest that the cardioprotective effects of both GLP-1 and GLP-1(9-36) are partially attributed to their vasodilatory mechanism which is nitric oxide(NO)/cGMP-dependent and partially to the enhanced stability of stromal cell-derived factor-1 alpha (SDF-1α), prolonging its lifetime and its physiological action involving recruitment of Endothelial progenitor cells (EPCs) from the bone marrow. EPCs contribute to the endothelialization of injured vessels and the neovascularization induced by ischemic events. One of the aims of the following Group study was to assess the effect of sitagliptin on reversing/attenuating the endothelial dysfunction by an insulin secretion independent mechanism in an animal model of type 1 diabetes, i.e., induced by STZ diabetes in Wistar rats. Diabetes was induced with a single intraperitoneal injection of streptozotocin (65 mg/kg in 10 mM citrate buffer, pH 4.5) to male Wistar rats (8 weeks old). After 2 weeks of diabetes induction the animals were divided into four groups: controls, diabetics and controls and diabetics treated with sitagliptin 5 mg/kg/day during the following 2 weeks. Rats were then anaesthetized (2 mg/kg intraperitoneal cocktail of a 2:1 (v/v) 50mg/mL ketamine solution in 2.5% chlorpromazine) and blood samples from the jugular vein were collected for the measurement of serum glucose, insulin and glycosylated hemoglobin (HbA1c) levels and measurement of DPP-IV activity. The animals were then sacrificed and three animals of each group were used for aorta isolation. Aorta rings were prepared for two cumulative concentration-response (CR) isometric relaxation curves to acetylcholine (ACh, 0.01 to 90 μM) after precontraction with 10 μM noradrenaline (NA). To confirm the NO production induced by ACh, 250 μM NG-nitro-L-arginine methylester (L-NAME), an inhibitor of endothelial NO synthase (eNOS) was added 30 min before the second CR curve, confirming the NO production induced by Ach in all four animal groups. ACh was more potent in inducing vasorrelaxation in control rats treated with sitagliptin (pEC50 of 6.77±0.09, n=12 vs. untreated control rats pEC50 of 6.41±0.14, n=12, p=0.039). Unexpectedly, in diabetic treated rats, sitagliptin seemed to compromise the NO-mediated vasorrelaxant effect ofACh with a significant reduction in the percentage of inhibition of NA-induced contraction (Emax of 27.1±5.1%, n=12 vs. untreated diabetic rats Emax of 59.5±8.2%, n=12, p=0.003). The technique of Western blotting was used to study the mechanisms underlying the results obtained previously. Thus, it was analysed the expression of GLP-1R and the total and phosphorylated Akt and eNOS proteins in order to assess the status of the signalling pathway Akt/eNOS in the presence and absence of sitagliptin in diabetic and control rats. In diabetic rats it was observed a significant increase in the expression of GLP-1R compared to control animals. In the group of diabetic rats, subjected to treatment with sitagliptin, there seems to be a tendency to approach the control situation with a reduced expression of GLP-1R. With regard to the expression levels of active Akt, determined by the ratio p-Akt/Akt, there was no statistically significant differences between groups. However, the administration of sitagliptin tend to increase the ratio of p-Akt/Akt in both control and diabetic rats, even though the latest showed a more tenuous increase. Finally, no statistically significant differences were observed in the active eNOS expression levels among all four study groups, determined by the ratio p-eNOS /eNOS. Oxidative stress markers were used in order to assess the presence of superoxide anions in the aorta of 4 groups of animals rings. There was an increase in oxidative stress in the aorta of diabetic rats rings, translated by an increase in fluorescence intensity compared to the respective control animals. The administration of sitagliptin decreased significantly fluorescence intensity, both in control animals or in diabetics. Finally, in order to complement these studies, histochemical studies were performed to characterize the micro-anatomy of the aortic rings 4 groups of animals. The histological changes observed in the present study demonstrated that the structural organization of the aortic tissue was disrupted in STZ induced diabetic rats, having the administration of sitagliptin in these animals resulted in an increase in the density of the matrix, in an attempt to approach the control the situation. To sum-up, these results show us that in the presence of diabetes, an impairment of the signaling pathway PI3K/Akt/eNOS may occur, leading to the development of diabetic endothelial dysfunction, characterized mainly by abnormalities in NO mediated vasodilation associated with oxidative stress. In fact, the beneficial effects of sitagliptin in the vascular function observed in the control group in this study, which are independent of insulin secretion, were not observed in the presence of pathology. This might be related to underlying phenomena that compromise thecommon signaling pathway and are not drug reversed at least in the short period of treatment the animals were subjected.
