Abstract: A síncope é um sintoma que consiste numa perda transitória da consciência com perda do tónus postural e tem como mecanismo subjacente uma hipoperfusão cerebral global momentânea.1 Trata-se de um problema clínico muito prevalente. Tem sido referido que mais de um terço dos indivíduos terá pelo menos um episódio sincopal ao longo da sua vida. Constitui uma importante causa de admissão hospitalar urgente e a taxa de recorrência é elevada.2 Apesar dos episódios sincopais serem frequentemente benignos e autolimitados, podem, por outro lado, ter um impacto negativo considerável na qualidade de vida, resultar em importante trauma físico e em acidentes ou associarem-se mesmo a um aumento da mortalidade.3 A síncope tem ainda implicações económicas significativas, contribuindo para tal, aspetos como o absentismo profissional, as frequentes admissões urgentes e internamentos hospitalares, e o recurso a múltiplos e variados exames complementares, nem sempre solicitados de forma criteriosa.4 A síncope é habitualmente classificada do ponto de vista fisiopatológico podendo ser atribuída a mecanismos reflexos (síncope neuromediada ou reflexa), a hipotensão ortostática ou a causas cardíacas (arritmias ou doença estrutural). Raramente está presente uma causa cerebrovascular (síndromes de roubo vascular).5 Esta patologia representa um problema comum que afeta todos os escalões etários, tendo, contudo, maior incidência, morbilidade e mortalidade na população geriátrica.6 O diagnóstico etiológico é importante para permitir um tratamento apropriado e definir o prognóstico. Apesar dos avanços que se têm registado na abordagem diagnóstica e estratificação de risco, o mecanisituações permanece por esclarecer, a causa nem sempre é óbvia e os indivíduos em risco nem sempre são fáceis de detetar. Na população idosa, onde a síncope é mais frequente, estas questões atingem ainda maior relevância, contribuindo para tal aspetos como a natureza por vezes atípica da apresentação clínica, a presença de amnésia, a dificuldade em distinguir os episódios de simples quedas, a existência de múltiplas comorbilidades ou de politerapia farmacológica, entre outros.Syncope is a symptom which consists of transient loss of consciousness with loss of postural tone and it has, as its underlying mechanism, a momentary global cerebral hypoperfusion.1 It is a very prevalent clinical problem. It has been reported that over one third of the individuals will have at least one syncopal episode during their lifetime. It is a major cause of urgent hospitalization and recurrence rate is high.2 Although syncopal episodes are often benign and self-limited, they can have a significant negative impact on the quality of life, result in major physical trauma and accidents or be associated with high mortality.3 Syncope still has significant economic implications, contributing aspects such as the professional absentism, frequent emergency admissions and hospital stays, and the use of multiple and various laboratory tests, not always ordered judiciously.4 Syncope is usually classified from a pathophysiological point of view and can be attributed to reflex mechanisms (neutrally-mediated or reflex syncope), orthostatic hypotension or cardiac causes (arrhythmias or structural disease). Rarely, it has a cerebrovascular cause (vascular steal syndrome).5 This condition is a common problem affecting all ages, with, however, a higher incidence, morbidity and mortality in the geriatric population.6 The etiological diagnosis is important to allow appropriate treatment and determine prognosis. Despite the advances that have occurred in the diagnosis and risk stratification, the mechanism of syncope in many situations remains unclear, the cause is not always obvious and individuals at risk are not easily detected. In the elderly population, where this clinical entity is more frequent, those issues can even be more complex, due to the oftenatypical clinical presentation, frequent amnesia, difficulty in distinguishing syncopal episodes from simple falls, the existence of multiple comorbidities or pharmacological polytherapy among other possible causes.
