Title: Avaliação do perfil demográfico cirurgico e de complicações de doentes operados com artroplastia total do joelho no serviço de ortopedia dos HUC

Abstract: Introdução: A artroplastia total do joelho é atualmente considerada um procedimento cirúrgico comum e em crescendo, acompanhando o envelhecimento da sociedade ocidental, onde a patologia osteoarticular é, também, mais prevalente. O objetivo deste estudo consiste em caracterizar uma amostra de doentes que foi submetida a Artroplastia Total do Joelho, bem como a morbimortalidade associada a este procedimento. Propõe-se, também, destacar os principais fatores que nos podem pôr na pista de maior probabilidade de insucesso cirúrgico e que influenciam a taxa de revisão. Métodos: Promoveu-se um estudo retrospetivo de doentes submetidos a Artroplastia Total do Joelho Primária no CHUC, operados no período compreendido entre 1 de janeiro de 2009 e 1 de janeiro de 2014. Os dados recolhidos através da consulta do processo clínico individual de cada doente foram introduzidos de forma anónima em folha Excel e a análise estatística foi realizada com o programa IBM SPSS Statistics. Resultados: Foram estudados um total de 372 doentes, 29 dos quais intervencionados duas vezes no período estudado, totalizando 401 cirurgias primárias. O sexo feminino esteve representado em 66,33% e o sexo masculino em 33,67%. A média de idades aquando da cirurgia primária foi de 68 ± 10,96 anos. O diagnóstico pré-operatório foi dominado pela artrose (90,77%). A presença de comorbilidades foi identificada em 83,29%, sendo as mais prevalentes a Hipertensão (68,83%), Dislipidémia (35,16%), Diabetes (19,7%) e Obesidade (19,45%). Os dois modelos de prótese mais utilizados foram PFC Sigma (56,61%) e Vanguard (26,68%), tendo o componente patelar sido mantido em 78,3% dos casos. Surgiram 48 casos de complicações, sendo as mais frequentes descolamento assético (31,25%), infeção (29,17%) e dor (18,75%). Foi registado apenas um óbito, o que traduz numa taxa de 5 mortalidade de 0,25%. Não foi encontrada diferença significativa entre fatores de risco e a existência de complicações (P=0,763). Conclusão: Apesar dos excelentes outcomes funcionais associados a este procedimento, não é possível, nem ético, negligenciar os aproximadamente 1/5 dos doentes que fica insatisfeito. Desde cedo se depreendeu que este sucesso estaria em clara dependência de muito mais do que fatores inerentes à própria cirurgia, pelo que ainda há muito a fazer no sentido de identificar as variáveis preditoras desses piores resultados. Adicionalmente, e dada a incapacidade de desenvolver conhecimento prático à mesma velocidade que novos avanços tecnológicos se vão tornando a realidade do presente, urge fomentar o hábito do registo individual de cada doente submetido a esta cirurgia, para que se possa proceder à transmissão de conhecimentos e experiência adquirida ao longo dos anos, com vista à melhoria da qualidade dos resultados e, sobretudo, à melhoria da qualidade de vida dos doentes
Introduction: Total Knee Arthroplasty is currently considered a common surgical procedure and in constant growth, along with the aging of the western society, where the osteoarticular pathology is also more prevalent. The aim of this study is to characterize a sample of patients who underwent Total Knee Arthroplasty, as well as the morbidity and mortality associated with this procedure. Our goal is also to highlight the main factors that can be associated with higher probability of surgical failure and, consequently, influence the revision rate. Methods: We promoted a retrospective study of patients who underwent Primary Total Knee Arthroplasty in CHUC that were operated in the period between January 1st of 2009 and January 1st of 2014. The data were collected through the consultation of the individual clinical process of each patient, introduced anonymously in an Excel sheet and the statistical analysis was performed with the program IBM SPSS Statistics. Results: We studied a total of 372 patients, 29 of whom operated twice during the study period, totaling 401 primary surgeries. Females were represented in 81.4% and males in 33.67%. The average age at the time of the primary surgery was 68 ± 10.96 years. The preoperative diagnosis was dominated by arthrosis, accounting for 364 cases (90.77 %). The presence of comorbidities was identified in 83.29%, being the most prevalent Hypertension (68.83%), Dyslipidemia (35.16%), Diabetes (19.7%) and Obesity (19.45%). The two prosthesis most commonly used were PFC Sigma (56.61 %) and Vanguard (26.68 %), with the patellar component being maintained in 78.3% of all cases. We identified 48 cases of complications, being the most frequent aseptic loosening (31.25%), infection (29.17%) and pain (18.75%). Only one death was registered as a direct result of the surgery, which translates in a mortality rate of 0.25 %. No significant difference was found between risk factors and the presence of complications (P= 0,763). 7 Conclusion: Despite the excellent functional outcomes associated with this procedure, it is not possible, nor ethical, to neglect the approximately 1/5 of the patients who is dissatisfied. Since early stages it was believed that this success would be in clear dependence of much more than factors inherent to the surgery itself, by which there is still much to be done in order to identify the predictors of these worst results. In addition, and given the inability to develop practical knowledge at the same rate as technological advances are becoming the new reality, it is also necessary to promote the habit of the individual record of each patient undergoing this surgery, so that it becomes possible to spread the knowledge and experience gained over the years, aiming at the improvement of the quality of the results and, in particular, the quality of life of patients
