Abstract: As queratoses actínicas (QAs) são lesões cutâneas comuns que surgem em áreas cronicamente expostas à radiação ultravioleta (UV). Podem regredir espontaneamente, permanecer estáveis ou progredir para carcinomas espinhocelulares (CECs). Dados recentes acerca da patogenia vieram alterar a abordagem desta patologia. Atualmente, a evolução de QA a CEC é vista como um continuum, o que se reflete na classificação da QA de acordo com sistema neoplasias queratinocíticas intraepidérmicas (KIN, do inglês). Para além disso, o conceito de cancerização de campo teve um importante impacto na terapia. De facto, a agressão pela radiação UV ocorre não só nos locais onde surgem as lesões individuais, como também em toda a área de pele foto-exposta, que representa o “campo”. Nestas zonas, é possível encontrar múltiplas QAs, lesões subclínicas e queratinócitos com alterações genéticas. Com base nesta teoria, as terapias foram divididas em focais, dirigidas a lesões individuais clinicamente identificáveis, e de campo, direcionadas a lesões clínicas e subclínicas presentes no campo de cancerização. Existem diversas opções terapêuticas disponíveis e novos agentes encontram-se em estudo. A combinação entre terapias tem também tido bons resultados. Contudo, não há consenso quanto à decisão de tratar ou não todas as lesões, nem quanto à escolha da modalidade terapêutica. Esta artigo de revisão tem como objetivo abordar a epidemiologia, patogenia, clínica, diagnóstico e terapia da QA, dando particular relevância aos recentes avanços na área da patogenia e suas implicações na terapêutica.
Actinic keratoses (AKs) are common skin lesions emerging from areas chronically exposed to ultraviolet (UV) radiation. AKs may regress spontaneously, remain stable or transform to invasive squamous cell carcinomas (SCCs). Recent findings regarding the pathogenesis have changed the management of AK. AK and SCC are nowadays seen as a continuum; this view is implied in the classification of AKs according to the keratinocytic intraepidermal neoplasias (KIN) system. Furthermore, one of the main principles underpinning the management of AK is that of field cancerization. In fact, the insult by the UV radiation targets not only the skin where the individual lesions develop, but also all the photo-exposed area, which represents the field. In this area, it is possible to find multiple AKs, subclinical lesions, as well as mutated keratinocytes. Based on this theory, therapies were divided into lesion-directed, aimed at individual AK lesions, and field-directed therapies, which are able to treat clinical and subclinical lesions in a cancerization field. There are several therapies currently available and novel therapies are under investigation. Combining therapies has had also good results. However, there is no agreement on whether or not to treat all the lesions, as well as on the choice of the therapy. This literature review article aims to describe the epidemiology, pathogenesis, clinical findings, diagnosis and therapy of AK, emphasizing the current understanding of pathogenesis and its implications in the therapy.
