Title: Correlação entre o risco anestésico pré-operatório e complicações locais e sistémicas em artroplastia total do joelho

Abstract: Introdução: A Artroplastia Total do Joelho (ATJ) é uma cirurgia cada vez mais frequente no tratamento da osteoartrose do joelho. A maioria dos doentes operados apresenta um número considerável de comorbilidades que devem ser avaliadas antes da cirurgia. A escala de risco anestésico da American Society of Anaesthesiologists (ASA) é um bom indicador da condição geral pré-cirúrgica podendo ser estudado para averiguar se um estado geral deficitário antes da cirurgia se pode refletir no aumento das complicações pós-operatórias. Objetivo: Pretende-se averiguar se existe correlação entre risco anestésico e complicações de ATJ surgidas até 1 ano após a cirurgia, e secundariamente correlacionar as condições peri-operatórias da cirurgia realizada com as principais complicações pós-cirúrgicas descritas como as mais frequentes na literatura médica. Metodologia: De forma aleatória foram identificados 100 doentes submetidos a ATJ entre 2010 e 2013 no polo HUC do Centro Hospitalar e Universitário de Coimbra. Levantados os respetivos processos clínicos do arquivo, procedeu-se de seguida à recolha dos dados, de forma anónima em base de dados, entre os quais os dados de risco do doente, sexo, idade, comorbilidades, avaliação de risco anestésico ASA e complicações pós-operatórias aí relatadas até um ano após a cirurgia. A avaliação estatística foi feita de forma descritiva recorrendo aos coeficientes de Pearson e Spearman para verificar a existência de correlação entre as condições peri-operatórias e as complicações estudadas. Resultados: Verificou-se a existência de 41% de infeções locais durante o seguimento de 1 ano na amostra avaliada. 44% dos doentes tiveram um internamento pós-operatório superior a 8 dias. Houve correlações positivas de intensidade média com significância estatística para p-value < 0,01 nos coeficientes de Pearson e Spearman entre o aumento da escala ASA e o surgimento de arritmia e disfunção cognitiva; demência e síndrome de apneia obstrutiva do sono (SAOS), disfunção cognitiva e arritmia; hemoglobina < 10 g/dl e albuminemia < 3,5 g/dl com insuficiência renal aguda; internamento pós-operatório superior a 8 dias e transfusão no mesmo período e o surgimento de infeção local. Conclusão: Foi patente o elevado número de infeções locais, nomeadamente relacionado com um internamento prolongado. Deve comparar-se a percentagem de infeções locais com outras unidades de saúde e instituir-se um rigoroso plano de assepsia e profilaxia antibiótica; o tempo de internamento prolongado deve ser combatido com a agilização das altas devido ao aumento do risco de infeções. O risco ASA tem de ser valorizado e recomenda-se a profilaxia antitrombótica e anticoagulante devido ao à correlação do seu valor com o surgimento de AVC e TEP. É válida e reprodutível a associação entre diversas condições pré-operatórias e respetivas complicações, destacando-se as seguintes e respetivas sugestões de atuação: a demência pré-operatória deve ser convenientemente avaliada antes da cirurgia, devendo estes doentes estar sob vigilância para deteção e tratamento precoce de SAOS, disfunção cognitiva e arritmia. Têm de ser valorizadas as alterações analíticas reveladoras de hipovolémia e repor o estado de hidratação do doente devido à sua correlação com insuficiência renal aguda.
Introduction: Total knee arthroplasty (TKA) is an increasingly used surgery to solve knee osteoarthrosis. Most of the intervened patients present a considerable number of comorbidities that should be taken into account before the surgical procedure. The anesthetic risk of the American Society of Anaesthesiologists (ASA) is an excellent indicator of pre-surgical condition that may be studied to determine if a poor general condition before surgery is reflected in the increase of postoperative complications. Objective: The aim, primarily, is to investigate the possible correlation between anesthetic risk and TKA complications that appear during the postoperative follow-up of at least 1 year and, secondly, to correlate perioperative conditions with most frequent postoperative complications described in the medical literature in the same period. Methodology: There were randomly collected 100 patients that underwent TKA between 2010 and 2013 in the Hospitais da Universidade de Coimbra (HUC), part of Centro Hospitalar e Universitário de Coimbra (CHUC). After collecting the respective medical records, we proceeded to the data collection, anonymously, in the data base, like the patients risk factors, genre, age, comorbidities, evaluation of the anesthetic risk and postoperative complications that the patients exhibit up until 1 year of follow-up. The statistical interpretation was descriptive statistics recurring to Pearson and Spearman coefficients to verify the correlation between conditions and the studied postoperative complications. Results: It was verified the existence of 41% local infections during follow-up. 44% of patients had a postoperative hospitalization higher than 8 days. There are positive correlations of medium intensity with a statistical significance p-value < 0,01 in Pearson and Spearman coefficients between increased ASA scale and the emergence of arrhythmia and cognitive impairment; dementia syndrome and obstructive sleep apnea (OSA), cognitive dysfunction and arrhythmia; hemoglobin < 10 g/dl and albumin < 3,5 g/dl with acute renal failure; postoperative hospitalization higher than 8 days and transfusion at the same time and the rise of the local infection. Conclusion: It was observed the high percentage of local infections, namely related with a prolonged hospitalization. It should be compared the percentage of local infections with other health units and to be performed a strict aseptic plan and antibiotic prophylaxis; prolonged hospital stay must be fought with earlier hospital discharge due to increased risk of infections. The ASA risk needs to be valued and it is recommended antithrombotic and anticoagulant prophylaxis due to the correlation of its value with the emergence of stroke and TEP. It is valid and reproducible the association between several pre-operative complications and respective complications, especially the following, and respective performance suggestions: preoperative dementia should be properly assessed before surgery - these patients should be under surveillance for detection and early treatment of OSA, cognitive dysfunction and arrhythmia; we must value the analytical data that refers to hypovolemia and restore the patient's hydration status due to its correlation with acute renal failure.
