Title: Adaptação para a população portuguesa de instrumentos de avaliação o impacto da saúde oral na qualidade de vida

Abstract: Objetivos: Os objetivos principais deste estudo passam pelo desenvolvimento de versões em língua Portuguesa das escalas Oral Health Impact Profile (OHIP-49), Oral Health Impact Profile-14 (OHIP-14) e Oral Health Impact Profile for assessing HRQoL in Edentulous Adults (OHIP-EDENT) e do questionário Denture Satisfaction Questionnaire (DSQ). De modo complementar pretende-se analisar a importância de variáveis sociodemográficas e subjetivas na auto-perceção da condição oral. Metodologia: Este trabalho seguiu os princípios recomendados na literatura para a adaptação de instrumentos construídos em contextos linguísticos e culturais diversos. A tradução dos instrumentos seguiu a metodologia tradução e retroversão. A validação foi obtida pela análise das propriedades psicométricas dos itens que compõem os instrumentos. Foi utilizado o alfa de Cronbach, como coeficiente de consistência interna, e o coeficiente ICC, como medida de confiabilidade. A validação de critério concorrente foi realizada através da associação entre o número de impactos funcionais e psicológicos e a autoavaliação da saúde oral. A validade de constructo foi avaliada através do método de máxima verosimilhança, considerando a matriz de covariâncias e adotando o método de estimação Maximum Likehood (ML), bem como pelo critério de Kaiser-Meyer-Olkin (KMO). Os instrumentos foram aplicados a 598 indivíduos, com uma média de idade de 60.27 anos. Resultados: Verificou-se um maior impacto da saúde oral, na população em geral, nas dimensões Limitação Funcional (9.85 ± 6.32) e Dor Física (9.29 ± 6.11), quando aplicada a escala OHIP-PT. Por outro lado, os resultados da escala OHIP-14-PT, demonstraram um maior impacto ao nível da Dor Física (2.39 ± 1.68) e Desconforto Psicológico (1.78 ± 1.90). No indivíduo desdentado, os resultados da escala OHIP-EDENT-PT, relevaram que as dimensões mais afetadas pela condição oral foram a Dor Física (5.11 ± 3.31) e a Limitação Funcional (4.94 ± 2.57). No que se refere à satisfação protética, aferida pelo QSP, os resultados revelaram uma cotação média superior para a arcada maxilar (15.33 ± 7.28), relativamente à arcada mandibular (14.25 ± 7.94). Obtiveram-se coeficientes alfa de Cronbach de 0.97 (OHIP-49), 0.94 (OHIP-EDENT e DSQ) e 0.93 (OHIP-14). O coeficiente de ICC registado, para os quatro instrumentos, foi de 0.99. As três escalas OHIP revelaram validade de critério concorrente (p<0.001). A análise fatorial confirmatória realizada para a OHIP-49 revelou índices de ajustamento sofríveis, ao passo que se registaram índices bons e muito bons para as escalas OHIP-14 e OHIP-EDENT. Na análise fatorial do DSQ, 2 fatores explicaram 86.65% da variação total, a medida de KMO foi de 0.87 e o teste de esfericidade de Bartlett foi 5384.00 com 66 graus de liberdade (p<0.001). Relativamente aos dados sociodemográficos, destaca-se a predominância do sexo feminino (64.05%) e salienta-se o equilíbrio percentual relativo de elementos de ambos os sexos em cada grupo etário, bem como entre sexos e grupos etários. Ademais, verificou-se a independência das variáveis referentes aos cuidados de saúde oral e à auto-perceção e autoavaliação da condição oral relativamente ao sexo. Em contrapartida, verificou-se uma dependência das variáveis relacionadas com os cuidados de saúde oral em relação à idade. Este estudo revelou também que os indivíduos, de maneira geral, fizeram uma avaliação positiva da sua saúde oral e que aqueles que a retrataram como sendo “Má” apresentaram um maior impacto da condição oral na sua qualidade de vida. No que diz respeito ao recurso a tratamento médicodentário, mais de metade dos participantes (56.86%) referiu ter necessidade. Conclusões: Os instrumentos revelaram-se válidos para a mensuração da qualidade de vida e da satisfação protética na população Portuguesa, apresentando propriedades psicométricas bastante robustas. No entanto, aconselha-se a utilização da escala OHIP-14- PT, para a população geral, e da OHIP-EDENT-PT e do QSP, para o indivíduo desdentado.
Objectives: The main objective of this study is the development of Portuguese versions of the Oral Health Impact Profile (OHIP-49), Oral Health Impact Profile-14 (OHIP-14), Oral Health Impact Profile for assessing HRQoL in Edentulous Adults (OHIP-EDENT) and Denture Satisfaction Questionnaire (DSQ) scales. Analyzing the importance of sociodemographic and subjective variables on self-perception of oral condition is the additional objective. Methodology: This work followed the principles recommended in the literature for the adaptation of instruments built in various linguistic and cultural contexts. The translation of the instruments followed the translation and retroversion methodology. Validation was obtained by analysis of the psychometric properties of the items that form the instruments. It was used Cronbach’s alpha, as an internal consistency coefficient, and the ICC coefficient as a reproducibility measurement. The concurrent criterion validity was performed through the association between the number of functional and psychological impacts and self-assessment of oral health. Construct validity was assessed using the maximum likelihood method, considering the covariance matrix and adopting the Maximum Likehood (ML) method of estimation, as well as the criterion of Kaiser-Meyer-Olkin (KMO). The instruments were applied to 598 individuals, with an average age of 60.27 years. Results: General population had a greater impact of oral health on Functional Limitation (9.85 ± 6.32) and Pain (9.29 ± 6.11), when applied OHIP-PT scale. Moreover, the OHIP-14- PT scale demonstrated a greater impact on Pain (2:39 ± 1.68) and Psychological Discomfort (1.78 ± 1.90). On the edentulous subject, the OHIP-EDENT-PT scale revealed that the most affected dimensions were Pain (5.11 ± 3.31) and Functional Limitation (4.94 ± 2.57). Prosthetic satisfaction, measured by QSP, revealed a higher median score for the upper maxillary arch (7:28 ± 3.33), compared to the mandibular arch (14:25 ± 7.94). Cronbach’s alpha coefficients of 0.97 (OHIP-49), 0.94 (OHIP-EDENT and DSQ) and 0.93 (OHIP-14) were obtained. The ICC coefficient noted for the four instruments was 0.99. The three OHIP scales revealed concurrent criterion validity (p<0.001). The confirmatory factor analysis for the OHIP- 49 showed tolerable adjustment indices, while there were good and very good rates for the OHIP-14 and OHIP-EDENT scales. In the factor analysis of the DSQ, 2 factors explained 86.65% of the total variation, the KMO measure was 0.87 and Bartlett’s sphericity test was 5384.00 with 66 degrees of freedom (p<0.001). Demographic data showed a predominance of females (64.05%), relative percentage balance of male and female elements in each age group and between genders and age groups. Moreover, there was an independence of variables related to oral health care and self-perception and self-assessment of oral condition in relation to sex. In contrast, there was a dependence on variables related to oral health care in relation to age. This study also found that, in general, people made a positive assessment of their oral health and that those who reported it as poor had a greater impact of oral condition on their quality of life. As regards to dental care, more than half of the participants (56.86%) indicated need. Conclusions: The instruments showed very robust psychometric properties and have proved to be valid for quality of life and prosthetic satisfaction measurement in the Portuguese population. However, it is advisable to use the OHIP-14-PT for the general population, and the OHIP-EDENT and DSQ for edentulous individuals.
