dc.description.abstractHemoglobinopatias devidas a mutações no gene β-globínico são responsáveis por doenças monogénicas muito comuns no mundo, nomeadamente drepanocitose e β-talassemia. A forma de hemoglobina presente em maiores concentrações no adulto (cerca de 95%) é a hemoglobina A (HbA; α2β2), estando presentes em menor percentagem a hemoglobina A2 (HBA2; α2δ2) (2-3,5%) e a hemoglobina fetal (HbF; α2γ2) (<2%). Durante o desenvolvimento fetal e primeiros meses de vida, a hemoglobina que predomina é a HbF. Em alguns doentes com hemoglobinopatias por mutações no gene β-globínico, nomeadamente na drepanocitose e β-talassemia, verifica-se um aumento dos níveis de HbF. A síntese aumentada de cadeias gama globínicas também persiste para além do nascimento em alguns adultos com parâmetros hematológicos normais, levando a que os níveis de HbF permaneçam elevados durante toda a vida; esta condição é conhecida como persistência hereditária de HbF (HPFH). O estudo da regulação dos níveis de HbF nos glóbulos vermelhos tem elevada importância clínica, na medida em que a persistência de HbF em doentes com drepanocitose e β-talassemia se traduz num benefício clínico importante, pois a sua presença compensa a redução de outras hemoglobinas funcionais. Através de estudos de associação, foram identificados três loci principais envolvidos na regulação da expressão dos genes gama: o gene BCL11A no cromossoma 2p16, a região intergénica HBS1L-MYB (HMIP) no cromossoma 6q23, e o SNP XmnI na região promotora do gene HBG2 no cromossoma 11p15.O presente estudo teve como objetivo avaliar a associação aos níveis de HbF de alguns polimorfismos previamente descritos nestes três loci em diferentes grupos populacionais de indivíduos de naturalidade Portuguesa normais e com HPFH e em doentes com β-talassemia. Também foi analisado um grupo de indivíduos de origem Africana com drepanocitose. A análise efetuada mostrou uma forte associação dos SNPs BCL11A (rs1188868 e rs766432) aos níveis de HbF (p=7,30x10-5 e p=0,002, respetivamente) no grupo populacional de indivíduos normais e com HPFH (n=60). No grupo com β-talassemia minor (n=42) foi encontrada uma forte associação do locus XmnI (p=2,267x10-6) aos níveis de HbF, bem como do SNP BCL11A rs766432 (p=0,001). Para os doentes com β-talassemia intermédia (n=18) e com drepanocitose (n=16) não foram obtidos valores significativos de associação aos níveis de HbF para qualquer um dos SNPs estudados.por
dc.description.abstractHemoglobinopathies due to mutations in the β-globin gene are responsible for monogenic diseases very common in the world, including sickle cell disease and β-thalassemia. The form of hemoglobin present in higher concentrations in the adult (about 95%) is hemoglobin A (HbA; α2β2); present in minimal percentages are hemoglobin A2 (HbA2; α2δ2) (2-3.5%) and fetal hemoglobin (HbF; α2γ2) (<2%). During fetal development and the first months of life, the predominant hemoglobin is HbF. Some patients with hemoglobinopathies due to mutations in the β-globin gene, in particular patients with sickle cell anemia and β-thalassemia, have high levels of HbF. The increased synthesis of gamma globin chains also persists beyond birth in some adults with normal hematological parameters leading that the HbF levels remain high throughout life; this condition is known as hereditary persistence of HbF (HPFH). The study of the regulation of HbF levels in red blood cells has high clinical importance. In fact, the persistence of HbF in patients with sickle cell anemia and β-thalassemia translates into a significant clinical benefit, because its presence compensates the reduction of other functional hemoglobins. Through association studies, three major loci were identified involved in the regulation of the gamma gene expression: the BCL11A gene on chromosome 2p16, the HBS1L-MYB intergenic region (HMIP) on chromosome 6q23, and the XmnI SNP in the promoter region of the gene HBG2 on chromosome 11p15. The present study aimed to evaluate the association of some polymorphisms previously described in these three loci with HbF levels, in different populations of vii individuals of Portuguese origin, namely in a normal and HPFH population group, and in patients with β-thalassemia. A group of patients of African origin with sickle cell anemia was also analyzed. Our study showed strong associations of BCL11A SNPs (rs1188868 and rs766432) with HbF levels (p = 7.30 x10-5 and p = 0.002, respectively) in the population group of normal and HPFH individuals (n = 60). In the group of patients with β-thalassemia minor (n = 42), strong association with HbF levels was found for the XmnI locus (p = 2.267 x10-6) and for the BCL11A SNP rs766432 (p = 0.001). In patients with β-thalassemia intermedia (n = 18) and sickle cell anemia (n = 16) no significant associations with HbF levels were found for any of the analysed SNPs.por
