Título: Apexificação e Revascularização pulpar em dentes permanentes imaturos: estudo experimental in vivo

Resumo: Durante o processo de desenvolvimento dos dentes permanentes é bastante comum a ocorrência de situações de cárie ou de traumatismos, que podem alterar ou mesmo danificar o tecido pulpar e prejudicar a fisiologia do complexo pulpo-dentinário e o normal desenvolvimento radicular. A perda prematura de uma polpa funcional em dentes imaturos leva à paragem da formação de dentina radicular, condicionando uma parede canalar pouco espessa e funcionalmente comprometida. Nestas circunstâncias clínicas, é geralmente recomendado um procedimento de apexificação com indução de uma barreira apical capaz de reter os materiais obturadores e impedir o seu contacto com os tecidos periapicais. Contudo nos dentes imaturos sujeitos à técnica de apexificação, mesmo quando tratados de acordo com o estado da arte, ou seja, barreira apical de MTA seguida de restauração adesiva, o prognóstico é incerto, devido à desfavorável relação coroa/raiz e grande suscetibilidade à fratura. Por outro lado, os procedimento de apexificação restringem a progressão e a maturação radicular. Com esta perspetiva, surgiram nos últimos anos alguns tratamentos inovadores que possibilitaram uma revascularização e revitalização do espaço pulpar de dentes imaturos necrosados e periodontite apical, permitindo o reforço das paredes radiculares e por vezes a continuação do seu desenvolvimento, abrindo assim novas possibilidades terapêuticas neste campo. É neste contexto que se situa o trabalho experimental aqui apresentado. Com efeito, o objetivo principal deste estudo consiste, para além da indução de um coágulo sanguíneo, numa avaliação in vivo do desempenho biológico e potencial regenerativo de diferentes matrizes (ácido hialurónico/quitosano e pectina/quitosano) colocadas no espaço canalar de dentes imaturos com necrose pulpar e periodontite apical, tendo por base análises radiográficas, histológicas, imunocitoquímicas e histomorfométricas. Na sua realização foram utilizados quatro cães da raça Beagle, envolvendo no total 56 dentes imaturos. Em cada um dos animais, foram selecionados 14 pré-molares. Estes dentes foram divididos em 6 grupos, nos quais estão incluídos dois grupos de controlos (20 canais) e quatro grupos de teste (76 canais) perfazendo um total de 96 canais radiculares. Os quatro grupos teste foram agrupados do seguinte modo: grupo 1 (apexificação com MTA); grupo II (coágulo sanguíneo); grupo III (coágulo sanguíneo + matriz polimérica de hialuronato de sódio/ quitosano – HA:CS) e grupo IV (coágulo sanguíneo + matriz polimérica de pectina/ quitosano - P:CS). Nos grupos controlo está inserido um controlo positivo (sem qualquer intervenção) e um negativo (infetados, sem qualquer tratamento). Nos resultados de apexificação (grupo 1), com base numa análise radiográfica foi possível verificar uma resolução das lesões periapicais (94%), um ligeiro aumento da espessura das paredes radiculares (77,8%) e encerramento apical (67%), apesar de uma ligeira extrusão do MTA (44%). Já as observações histológicas mostraram o lúmen dos canais completamente preenchidos com MTA, bem como a criação de uma barreira apical formada maioritariamente por cemento celular. Nos grupos experimentais em que foram aplicadas as matrizes (grupos 2, 3 e 4), foi possível confirmar radiograficamente, na grande maioria dos casos, a existência de uma barreira cervical de MTA adequada e bastante satisfatória. Verificou-se também uma evolução positiva no sentido da resolução da radiotransparência apical em 84%, um aumento da espessura das paredes radiculares em 77,2 % e um encerramento apical parcial e/ou completo em 70,2% das situações estudadas. A análise dos resultados em microscopia de luz revelou que os procedimentos de revascularização aplicados em dentes imaturos com necrose pulpar e periodontite apical, permitiram a recuperação de um estado vital no canal pulpar, possibilitando um desenvolvimento radicular para apical da restauração, traduzido por um aumento, ainda que mais significativo em espessura (85,6%) do que em comprimento (45,6%), das paredes radiculares, a par de grande número de casos (66,7%) de encerramento apical. Independente das matrizes utilizadas encontrámos com grande frequência, o espaço canalar novamente preenchido com grandes áreas de tecido conjuntivo laxo muito vascularizado e a presença de tecidos mineralizados com diferentes características e localizações. De entre estes tecidos destaca-se por um lado a formação de cemento celular em aposição à face interna da dentina, geralmente acompanhado por um tecido conjuntivo semelhante ao ligamento periodontal (contendo também alguns restos epiteliais de Malassez), bem como várias áreas de tecido ósseo, por vezes bastante extensas e dispersas no lúmen do canal. O conjunto destas estruturas no espaço canalar, parece resultar da migração e invasão dos tecidos constituintes do periodonto para o interior do canal pulpar, situação certamente facilitada pela dimensão do ápice. Apesar da possibilidade teórica de uma maior eficácia nos processos de regeneração com aplicação de diferentes matrizes (grupo 3 e 4), foram encontrados melhores resultados apenas com a indução, formação e estabilização de um coágulo sanguíneo (grupo 2). Assim, a capacidade regenerativa resultante da adição das matrizes ao coágulo não se revelou favorável. Um aspeto que nos parece importante registar é a presença quase constante de formações mineralizadas, circundadas por ligamento periodontal, contendo restos epiteliais de Malassez, situadas a uma certa distância das raízes dentárias correspondentes. Estas áreas de tecido mineralizado são essencialmente constituídas por cemento celular e dentina, mostrando uma composição em tudo semelhante às paredes do ápice de qualquer raiz, incluindo a presença de foramina, surgindo como resultado de um processo de diferenciação e maturação da papila apical. O aparecimento isolado destas estruturas, ou seja a sua separação da raiz correspondente, pode ficar a dever-se a traumatismos mecânicos ou factores iatrogénicos, tendo em conta que estas formações são facilmente destacáveis. A preservação da integridade estrutural da papila apical (evitando também o seu destacamento da raiz correspondente) e da bainha radicular epitelial de Hertwig (ou dos seus derivados), bem como efetuar um diagnóstico atempado e controlo da infeção/inflamação, a par de um eficaz mecanismo de revascularização, são condições essenciais que, à partida, podem proporcionar o desenvolvimento e maturação das células estaminais, adequadas à diferenciação fenótipica dos tecidos a regenerar, o que pode fazer toda a diferença na reconstituição do complexo pulpo dentinário e crescimento radicular.
During the development of permanent teeth, the occurrence of caries or trauma is quite common and can alter or even damage the pulp tissue and impair the physiology of the pulp-dentin complex and the normal root development. The premature loss of a functional pulp in immature teeth leads to the stopping of the root dentin formation, creating a thin and functionally compromised canal wall. In these clinical circumstances, an apexification procedure is generally recommended, inducing an apical barrier capable of retaining the filling materials and preventing their contact with the periapical tissues. However, in immature teeth subject to an apexification technique, even when treated according to the state of the art, in other words, with an apical barrier of MTA followed by an adhesive restoration, the prognosis is uncertain due to the unfavourable crown-to-root ratio and high susceptibility to fracture. On the other hand, since the apexification procedure restricts the root maturation and progression, This is the context in which the experimental work presented here is founded. Indeed, the main objective of this study is, in addition to the induction of a blood clot, an in vivo evaluation of the biological performance and regenerative potential of different scaffolds (hyaluronic acid/chitosan and pectin/chitosan) placed within the root canals of immature teeth with pulp necrosis and apical periodontitis, based on radiographic, histological, immunocytochemical and histomorphometric analysis. In the study, four beagle dogs were used, involving a total of 56 immature teeth. In each animal, 14 premolars were selected. These teeth were divided into six groups, among which two control groups (20 canals) and four test groups (76 canals) are included, for a total of 96 root canals. The four test groups were grouped as follows: group 1 (apexification with MTA); group II (blood clot); group III (blood clot + polymeric scaffold of sodium hyaluronate and chitosan - HA:CS) and group IV (blood clot + polymeric scaffold of pectin and chitosan - P:SC). In the control groups, a positive control (no intervention) and a negative one (infected without any treatment) were inserted. In the apexification results (group 1), based on X-ray analysis it was possible to verify a resolution of periapical lesions (94%), a slight increase in root wall thickness (77,8%) and continued apical closure (67%), in spite of a slight extrusion of the MTA (44%). The histological observations showed the lumen of the root canal completely filled with MTA, as well as the creation of an apical barrier formed mainly by cellular cementum. In the experimental groups where the scaffolds were applied (group 2, 3 and 4), it was confirmed radiographically, in most cases, the existence of an adequate and quite satisfactory cervical MTA barrier. There was also a positive trend towards the resolution of the apical radiolucency in 84%, an increase in root wall thickness in 77,2% and a partial and/or total continued apical closure in 70,2% of the studied cases. The results analysis in light microscopy revealed that revascularization procedures applied in immature teeth with necrotic pulp and apical periodontitis (groups 2, 3 and 4) showed us the recovery of a vital state in the pulp canal, allowing root growth apically to the restoration, reflected by an increase, though more significant in terms of thickness (85,6%) than in length (45,6%), of the root canal walls, together with a large number of cases (66,7%) of apical closure. Regardless of the scaffolds, we very frequently found the canal space once again filled with large areas of very vascularized soft tissue and the presence of mineralized hard tissues with different characteristics and locations. Among these tissues stands out the formation of cellular cement in apposition to the inner surface of the dentin, often accompanied by a connective tissue similar to periodontal ligament (also containing some epithelial rests of Malassez) as well as several areas of bone tissue, sometimes quite extensive and dispersed in the lumen of the canal. All these structures within the canalar space appear to result from the migration and invasion of periodontal tissue constituents into the pulp canal, a situation certainly facilitated by the size of the apex. In spite of the theoretical possibility of a more effective regeneration processes with the use of different scaffolds (group 3 and 4), better results were found with only the induction, formation and stabilization of a blood clot (group 2). Thus, the regenerative capacity resulting from the addition of the scaffolds to the blood has not proved favourable. One aspect that seems important to note is the almost constant presence of mineralized formations, surrounded by periodontal ligament, containing epithelial rests of Malassez, located at a distance from corresponding tooth roots. These areas of mineralized tissue are essentially made of cellular cementum and dentin, showing a composition similar to the walls of any root apex, including the presence of foramina, arising as a result of a process of differentiation and maturation of the apical papilla. Bone trabeculae, in most cases, occupy the space between these configurations and the apical portion of the root. These structures appear to represent the development of an apex totally or partially separated from the remainder corresponding root, depending on the connection, or not, to root canal walls. In fact, it was observed, in the same root, an attempt to close one of the root walls (with cementum, dentin, and there is foramina), while the other remained open. This seems to be due to a continuation of the root wall with its apical papilla while the other lost this contact and, thus, did not benefit from the contribution of the constituent cells of the apical papilla to form the dentin layer of the root wall. The appearance of these structures, that is, its separation from their corresponding root, may be due to mechanical trauma and iatrogenic factors, taking into account that these formations are easily detachable. Preserving the structural integrity of the apical papilla (also avoiding its detachment from the corresponding root) and Hertwig’s epithelial root sheath (or its derivatives), as well as making timely diagnosis and control of infection/inflammation, along with an effective revascularization mechanism, are essential conditions that can provide the development and maturation of stem cells suitable for phenotypic differentiation of tissues to regenerate, which can make all the difference in the reconstitution of the pulpo-dentinal complex and root growth.
