Rutura e reparação da aliança terapêutica na terapia familiar sistémica : um estudo exploratório com clientes involuntários

Abstract

O presente estudo exploratório tem como principal finalidade analisar o processo de rutura-reparação da aliança terapêutica num caso clínico terminado com uma família involuntária. O recurso ao System for Observing Family Therapy Alliances (SOFTA; Friedlander, Escudero, & Heathrington, 2006) permitiu identificar e caracterizar repetidos marcadores de rutura na aliança na 1ª e 4ª sessões e 2ª sessão de follow-up, manifestados na demonstração de comportamentos de confrontação e afastamento, interações familiares problemáticas e uma aliança cindida com o sistematerapeutas. Comprovada a reparação da aliança nessas três sessões terapêuticas, procedeu-se à caracterização das intervenções dos terapeutas que contribuíram para o fortalecimento da aliança. A análise observacional indica que o processo de reparação mais bem-sucedido ocorreu durante a 4ª sessão e envolveu a discussão direta da rutura e a redefinição das metas terapêuticas. Os resultados demonstram que a intervenção do sistematerapeutas ao nível da promoção do envolvimento dos clientes na terapia teve um impacto significativo no processo de reparação da aliança terapêutica.

This exploratory study mainly aims to analyze the therapeutic alliance rupture-repair process in a finished clinic case with an involuntary family. The use of the System for Observing Family Therapy Alliances (SOFTA; Friedlander, Escudero, & Heathrington, 2006) allowed to identify and characterized repeated rupture markers during the 1st and 4th sessions and the 2nd follow-up session, manifested in confrontation and withdrawal behavior, problematic within-family interactions and a split aliance with the therapists. Proven the alliance repair in these three therapeutic sessions, we analyse the therapists’ interventions which contributed to the strengthening of the alliance. The observational analysis indicates that the most successful repair process took place during the 4th session and involved the direct discussion of rupture and the redefinition of therapeutic targets. The results demonstrate that the therapists’ contribution to the engagement of clients in therapy had a significant impact on the repair process of the therapeutic alliance.
