Abstract: A síndrome de blefarofimose-ptose-epicantus é um raro transtorno do desenvolvimento, de transmissão autossómica dominante, causada por mutações no gene FOXL2 ou por delecções génicas totais, translocações cromossómicas envolvendo o locus do gene ou perturbando os elementos distantes cis-reguladores do FOXL2. Esta síndrome craniofacial de penetrância completa apresenta duas variantes clínicas: Tipo I que associa alterações das pálpebras a insuficiência ovárica, e o tipo II com dismorfismo palpebral isolado. O nosso objectivo foi desvendar o defeito genético subjacente numa cohort de doentes portugueses com BPES combinada com a abordagem clínica global destas crianças. Seis doentes portugueses não aparentados com fenótipo BPES foram submetidos a uma avaliação fenotípica completa dos pontos de vista da genética clínica e oftalmológico, assim como de uma avaliação citogenética e molecular. Este estudo prevê a identificação das alterações subjacentes ao fenótipo de BPES a nível molecular e/ou citogenético nestes doentes, permitindo o estabelecimento de uma correlação genótipo-fenótipo, aconselhamento genético adequado e diagnóstico pré-natal. Esta pesquisa foi realizada em concordância com os princípios da Declaração de Helsínquia e com as guidelines institucionais definidas pelo comité de ética da Faculdade de Medicina de Coimbra.
Blepharophimosis-ptosis-epicanthus syndrome (BPES) is a rare autosomal dominant developmental disorder caused by FOXL2 mutations, total gene deletions, chromosomal translocations involving the gene locus or disrupting distant regulatory elements surrounding FOXL2. This craniofacial syndrome of complete penetrance presents two distinct clinical variants: type I that associates eyelid changes with ovarian insufficiency, and type II with isolated eyelid defects. Our aim was to unravel the underlying genetic defect in a cohort of Portuguese BPES patients combined with comprehensive clinical approach of these children. Six independent portuguese patients with the BPES phenotype underwent complete evaluation from a clinical genetics, ophthalmological, cytogenetics and molecular standpoints. This study envisions to the identification of the molecular and cytogenetic changes underlying BPES in this Portuguese cohort of patients, allowing the establishment of genotype-phenotype correlations, appropriate genetic counseling and prenatal diagnosis. This research was conducted in accordance with the tenets of the Declaration of Helsinki and with the institutional guidelines defined by the ethics committee of the Faculty of Medicine of Coimbra.
