This disease usually appears at birth, due to mutations in genes essential to the normal functioning of the skin barrier. These genetic alterations lead to mechanical disruption of structural proteins, influencing dermoepidermal integrity and adhesion. EB subtypes are characterized by mucocutaneous fragility, with the formation of vesicles and bullae usually induced by minimal mechanical trauma. EB can be categorized into four classic types: EB simplex, junctional EB, dystrophic EB and Kindler's EB, distinguished by clinical presentation, cleavage plane and molecular pathology. Despite their clinical heterogeneity, complaints such as pain and pruritus are common to most subtypes. This is a severe and complex multisystemic orphan disease that reduces both quality of life and average life expectancy. To date, therapeutic approaches remain limited and none have proven consistently effective. For this reason, their treatment is challenging and mostly supportive, focusing on symptoms and the prevention of lesions and complications, such as squamous cell carcinoma. Consequently, regular and rigorous multidisciplinary follow-up is essential. Recent and promising therapeutic approaches are fundamentally based on cell, gene and prognosis modifying therapies. This article focuses on updating the state of the art in recessive dystrophic EB, looking at treatments, both approved and under study, which have had beneficial effects, both clinically and histologically. A epidermólise bolhosa (EB) é uma genodermatose hereditária que engloba vários subtipos, clínica e geneticamente heterogéneos. Esta patologia surge, habitualmente, ao nascimento, sendo secundária a mutações em genes essenciais ao normal funcionamento da barreira cutânea. Estas alterações genéticas condicionam disrupção mecânica de proteínas estruturais, influenciando a integridade e a adesão dermoepidérmica. Os subtipos da EB caracterizam-se por fragilidade mucocutânea, com formação de vesículas e bolhas induzidas, geralmente, por trauma mecânico mínimo. A EB pode ser categorizada em quatro tipos clássicos: EB simplex (EBS), EB juncional (EBJ), EB distrófica (EBD) e EB de Kindler (EBK), distinguidos pela apresentação clínica, plano de clivagem e patologia molecular. Apesar da sua heterogeneidade clínica, queixas como a dor e o prurido são transversais à maioria dos subtipos. Esta é uma doença órfã multissistémica, grave e complexa, que provoca diminuição da qualidade de vida e da esperança média de vida. Até à data, as abordagens terapêuticas permanecem limitadas e nenhuma se revelou consistentemente eficaz. Por este motivo, o seu tratamento é desafiante e, maioritariamente, de suporte, focando-se nos sintomas e na prevenção de lesões e complicações, como o carcinoma espinhocelular (CEC). Desta forma, é essencial existir um seguimento multidisciplinar regular e rigoroso dos doentes. As abordagens terapêuticas recentes e promissoras baseiam-se, fundamentalmente, em terapias celulares, génicas e modificadoras de sintomas e prognóstico. O presente trabalho foca-se na atualização do estado da arte da epidermólise bolhosa distrófica recessiva (EBDR), abordando tratamentos, aprovados e em estudo, com efeitos benéficos, tanto a nível clínico, como a nível histológico. |
