Title: The ubiquitin-proteasome pathway, oxidative stress and age-related macular degeneration : the good, the bad and the ugly

Abstract: Age-related macular degeneration (AMD) is the leading cause of blindness in the western world in the population over 60 years of age and its prevalence is likely to rise as a consequence of increasing longevity. While the cause of AMD is currently not known, it has been suggested that multiple factors are involved including environmental, nutritional and genetic factors. Most of these risk factors appear to have oxidative stress as a common denominator. Recent findings further suggest a role for inflammation in the pathogenesis of AMD. However, the molecular mechanisms that associate oxidative stress to in AMD are still unclear. The ubiquitin-proteasome pathway (UPP) plays critical roles in various cellular functions, including signal transduction, transcription, fine regulation of the cell cycle and protein quality control. Previous studies have demonstrated that the UPP activity in retinal pigment epithelium (RPE) is modulated by cellular redox status. Moreover, we and others have demonstrated that oxidative stress can impair the activity of the proteasome. In this thesis we hypothesize that age-related impairment of the proteasome activity by increased oxidative stress is a mechanistic link that leads to altered expression of genes involved in angiogenesis and inflammation. This hypothesis would provide a new association between retinal oxidative insult and expression of AMD-like phenotypes, such as increased inflammation, accumulation of drusen (caused by decrease macrophage recruitment) and neovascularization of the retina. To assess the effect of oxidative stress on UPP activity, ARPE-19 cells were exposed to continuously generated H2O2 or A2E-mediated photooxidation. Exposure of ARPE-19 cells to physiologically relevant sources of oxidative stress resulted in a 30% to 50% reduction in all three peptidase activities of the proteasome. Exposure to H2O2 or A2E-mediated photooxidation also resulted in an increased conjugation activity and a two- to three-fold increase in levels of endogenous ubiquitin conjugates. These data show that the proteasome in ARPE-19 is susceptible to oxidative inactivation, whereas activities of the ubiquitin-conjugating enzymes (E1) are more resistant to oxidative stress. Stress-induced overproduction of inflammatory cytokines, such as interleukin- 8 (IL-8), is one of the early events in a number of age-related diseases, including AMD. However, the molecular mechanism underlying this enhanced production of IL-8 is still unclear. Therefore, we aimed at elucidating mechanistic links between oxidative stress and overproduction of IL-8 in RPE cells. We found that exposure of RPE cells to H2O2, paraquat, or A2E-mediated photooxidation resulted in increased expression and secretion of IL-8. All of these oxidative stressors also inactivated the proteasome in RPE cells. In contrast, tert-Butylhydroperoxide (TBH), a lipophilic oxidant, did not stimulate IL-8 production, nor did it inactivate the proteasome. Moreover, prolonged treatment of RPE cells with proteasome-specific inhibitors recapitulated the stimulation of IL-8 production. Proteasome inhibition both attenuated the activation and delayed the turnoff of the transcription factor NF-κB, resulting in biphasic effects on the production of IL-8. Prolonged proteasome inhibition (>2 h) resulted in activation of p38 MAPK via activation of MKK3/6 and increased the production of IL-8. By chemically inhibiting p38 MAPK it was possible to prevent the up-regulation of IL-8 in response to proteasome inhibition. We have further elucidated a novel molecular mechanism downstream of p38 MAPK underlying the up-regulation of IL-8 induced by proteasome inhibition. Proteasome inactivation in retinal pigment epithelial cells led to p38 MAPK, EGFR and PI3K activation. Inhibition of any of these pathways abolished the enhanced production of IL-8 following proteasome inhibition. Using chemical inhibitors and constitutively active and dominant negative forms of MKK3, MKK6 and Akt, we further elucidated a cascade of sequential activation of these signaling pathways in response to proteasome inhibition. This cascade is initiated by proteasome inhibition and subsequently involves activation of MKK3/MKK6, EGFR phosphorylation, PI3K activation and increased IL-8 expression. Furthermore, this work identifies the Itk kinase as a novel regulator of IL-8 expression upon proteasome inhibition. In addition to the inflammatory component, choroidal neovascularization and impaired macrophage recruitment are also key features of AMD. Consistently, we demonstrated that inhibition of UPP in RPE stabilized HIF-1α and increased levels of mRNA for VEGF and angiopoietin-2 (Ang-2) up to 7-fold. Proteasome inhibition was also associated with a 2-fold increase in the levels of mRNA for angiopoietin-1 (Ang- 1) and with an increase in the secretion of VEGF by 2-fold. In contrast, the expression and secretion of MCP-1 by RPE were substantially attenuated as a result of a decrease in NF-κB activation caused by inactivation of the proteasome. Taken together, the results presented in this thesis strongly suggest that the oxidative impairment of the UPP in RPE is a major mechanistic step that associates increased oxidative stress during aging to the development of AMD. By understanding the role of UPP in the RPE and its contribution to AMD it might be possible to develop new animal models of AMD that may facilitate an understanding of the molecular mechanisms of the disease. On the other hand, by identifying the proteasome as a target of oxidative stress upon aging, it might be possible to envision innovative therapeutic opportunities for AMD based on the “protection” of the proteasome.
A degenerescência macular relacionada com a idade (DMRI) é a principal causa de cegueira na população com idade superior a 65 anos em países industrializados, sendo de esperar que a sua prevalância aumente em consequência do aumento da esperança média de vida das populações. Apesar dos mecanismos moleculares associados à DMRI não serem ainda bem conhecidos, o stress oxidativo parece desempenhar um papel fundamental no desenvolvimento e progressão da doença. Alguns estudos recentes sugerem ainda que esta patologia apresenta uma importante componente inflamatória. No entanto, a relação entre stress oxidativo e inflamação na DMRI não é ainda clara. A via da ubiquitina-proteassoma (VUP) tem um papel fundamental na regulação celular, participando virtualmente em todos os processos críticos que asseguram a sobrevivência celular. Neste estudo sugere-se que a inactivação de componentes críticos da VUP no EPR pode contribuir para alterações celulares e fenotípicas tipicamente associadas à DMRI, incluindo a neovascularização e a componente inflamatória. Esta disfunção da VUP poderá, por isso, constituir um novo mecanismo que permite associar a lesão oxidativa da retina à expressão de características fenotípicas próprias da DMRI, nomeadamente o aumento da produção de mediadores inflamatórios, acumulação de drusen (devido à diminuição no recrutamento de macrófagos) e neovascularização da retina. Para avaliar o efeito do stress oxidativo na actividade da VUP, uma linha celular do EPR (ARPE-19) foi exposta a um fluxo contínuo de peróxido de hidrogénio ou a fotooxidação mediada por A2E. A exposição das células a agentes de lesão oxidativa fisiologicamente relevantes levou a uma diminuição de 30% a 50 % nas três actividades proteolíticas do proteassoma. A exposição a peróxido de hidrogénio ou fotooxidação mediada por A2E conduziu ainda a um aumento significativo na actividade de conjugação de ubiquitina e a uma acumulação de conjugados de ubiquitina endógenos. Estes resultados demonstram que o proteassoma é um alvo de lesão oxidativa em células do EPR, ao passo que a actividade das enzimas conjugadoras de ubiquitina parece ser mais resistente à oxidação. O aumento da produção de citocinas inflamatórias, como a interleucina-8 (IL- 8), em resposta ao stress sugere a participação de uma componente inflamatória em doenças associadas ao envelhecimento. No entanto, o mecanismo molecular subjacente a este aumento não é, ainda, bem conhecido. Assim, um dos principais objectivos deste trabalho consistiu em identificar eventuais mecanismos moleculares através dos quais o stress oxidativo pudesse conduzir a um aumento na produção de IL-8 no EPR. Os resultados mostram que a exposição de células ARPE-19 a peróxido de hidrogénio, paraquat, ou fotooxidação mediada por A2E conduziu a um aumento na expressão e secreção de IL-8. Este aumento na produção de IL-8 foi acompanhado pela inactivação do proteassoma em células do EPR. Deve ainda referir-se que o tratamento prolongado de células do EPR com inibidores específicos do proteassoma reproduziu o aumento na produção de IL-8, observado na sequência da exposição das células a agentes oxidantes. A inactivação do proteassoma em células do EPR conduz também a uma redução na activação do factor de transcrição NF-B, prolongando simultaneamente a sua ligação ao DNA, produzindo um efeito bifásico na produção de IL-8. Este trabalho contribui ainda para a identificação de um novo mecanismo molecular, dependente da p38 MAP cinase e que contribui para o aumento na produção de IL-8 em resposta à inibição do proteassoma. As vias de sinalização activadas em resposta à inibição do proteassoma incluem activação das cinases MKK3/MKK6, fosforilação do receptor do factor de crescimento epidermal, activação da fosfatidilinositol-3-cinase e produção de IL-8. O tratamento de células do EPR com inibidores especifícos do proteassoma conduziu à activação da p38 MAP cinase, factor de crescimento epidermal e fosfatidilinositol-3-cinase. A inibição destas vias impediu o aumento na produção de IL-8 induzida por inibidores do proteassoma, o que é consistente com a hipótese da participação do proteassoma neste processo. Este trabalho identificou também a cinase Itk como um novo regulador da expressão da IL-8 induzida pela inibição do proteassoma. Para além da componente inflamatória, a neovascularização coroideia e a diminuição no recrutamento de macrófagos constituem duas importantes características da DMRI. Estas duas components fenotípicas da DMRI podem também ser explicadas com base na hipótese, aqui proposta, de que a inibição do proteassoma contribui para o desenvolvimento e progressão da doença. De facto, os resultados apresentados mostram que a inibição da VUP estabiliza o factor de transcrição HIF-1e aumenta os níveis de RNA do VEGF e angiopoietina-2 (Ang-2). A inactivação do proteassoma também aumentou a expressão da angiopoietina-1 (Ang-1) e a secreção de VEGF. Por outro lado, os níveis de MCP-1 (proteína quimiotáxica para monócitos) diminuíram em resposta à inibição do proteassoma, provavelmente devido à menor activação do NF-B. No seu conjunto, os resultados apresentados nesta tese permitem propôr um novo mecanismo através do qual a inactivação oxidativa do proteassoma no EPR contribui para o desenvolvimento e progressão da DMRI. A identificação da VUP como um alvo de stress oxidativo no EPR e o efeito da inactivação de componentes críticos da via no desenvolvimento e progressão da DMRI constitui uma contribuição importante deste estudo para uma melhor compreensão dos mecanismos moleculares associados à doença e ainda para o desenvolvimento de novas estratégias terapêuticas.
