Abstract: Background: Paediatric high grade glioma (pHGG) is a distinct biological entity to histologically similar tumours
arising in older adults, and has differing copy number profiles and driver genetic alterations. As functionally
important intragenic copy number aberrations (iCNA) and fusion genes begin to be identified in adult HGG, the
same has not yet been done in the childhood setting. We applied an iCNA algorithm to our previously published
dataset of DNA copy number profiling in pHGG with a view to identify novel intragenic breakpoints.
Results: We report a series of 288 iCNA events in pHGG, with the presence of intragenic breakpoints itself a
negative prognostic factor. We identified an increased number of iCNA in older children compared to infants, and
increased iCNA in H3F3A K27M mutant tumours compared to G34R/V and wild-type. We observed numerous gene
disruptions by iCNA due to both deletions and amplifications, targeting known HGG-associated genes such as RB1
and NF1, putative tumour suppressors such as FAF1 and KIDINS220, and novel candidates such as PTPRE and KCND2.
We further identified two novel fusion genes in pHGG – CSGALNACT2:RET and the complex fusion DHX57:TMEM178:
MAP4K3. The latter was sequence-validated and appears to be an activating event in pHGG.
Conclusions: These data expand upon our understanding of the genomic events driving these tumours and

Abstract: Introdução: Caracterizar as notificações espontâneas de eventos adversos a medicamentos recebidas pela Unidade de Farmacovigilância
do Centro.
Material e Métodos: Consideraram-se todas as notificações reportadas entre 01/2001 e 12/2013. Estimaram-se taxas de notificação
anuais. Os casos foram caracterizados quanto à gravidade, conhecimento prévio, causalidade imputada, origem e grupo profissional
do notificador, tipo de evento adverso e grupos farmacoterapêuticos onde se incluem os medicamentos suspeitos com maior prevalência
de notificação.
Resultados: A Unidade recebeu 2408 notificações, que continham 5749 eventos adversos. No ano de 2013 foi registada uma taxa
de notificação de 171 notificações/milhão de habitantes. Do total de notificações, 55% foram classificadas como graves. Das notificações
com causalidade imputada, 90% tinham uma relação pelo menos possível com o medicamento suspeito. Os medicamentos
que originaram maior número de notificações foram os anti-infeciosos para uso sistémico (n = 809; 33%), e os eventos adversos
mais frequentemente notificados foram as “Afeções dos tecidos cutâneos e subcutâneos” (n = 1139; 20%). Registaram-se 154 (6,4%)
casos de risco de vida e/ou morte e 88 (3,6%) continham pelo menos um evento adverso classificado simultaneamente como grave,
desconhecido e definitivo ou provável.
Discussão: Os resultados deste estudo são consistentes com os de outros estudos, designadamente no que diz respeito à gravidade,
aos grupos farmacoterapêuticos onde se incluem os medicamentos suspeitos e aos tipos de eventos adversos reportados.
Conclusão: Ao longo do período avaliado, a UFC solidificou a sua atividade, tendo verificado um crescimento da taxa de notificação
em geral e um aumento da notificação de reações adversas graves e desconhecidas; Introduction: The aim of this study was to characterize the spontaneous reports of adverse events that were received by the Central
Portugal Regional Pharmacovigilance Unit.
Material and Methods: Spontaneous reports received between 01/2001 and 12/2013 were considered. The annual reporting ratios
were estimated. The cases were characterized according to their seriousness, previous description, causality assessment, origin and
professional group of the reporter, type of adverse event and pharmacotherapeutic groups of the suspected drugs most frequently
reported.
Results: The Pharmacovigilance Unit received 2408 reports that contained 5749 adverse events. In 2013, the reporting rate was
estimated at 171 reports per million inhabitants. Fifty-five percent of the reports were assessed as serious. Ninety percent of the cases
were assessed as being at least possibly related with the suspected drug. The suspected drugs most frequently reported were antiinfectives
for systemic use (n = 809, 33%). The most frequently reported adverse events were “Skin and subcutaneous tissue disorders”
(n = 1139, 20%). There were 154 (6.4%) reports resulting in life-threatening situations and/or death, and 88 (3.6%) containing at least
one adverse event assessed as serious, unknown and certain or probable.
Discussion: The present results are in line with those found in other studies, namely the seriousness and type of the adverse events
and the pharmacotherapeutic groups of the most frequently reported suspected drugs.
Conclusion: In the last years, the Central Portugal Regional Pharmacovigilance Unit has registered a growth in the reporting rate in

Abstract: Introdução: A reconciliação terapêutica visa promover a segurança do doente por meio da redução de erros de medicação e eventos
adversos decorrentes de discrepâncias de medicação na transição de cuidados. Foi nosso objetivo realizar um estudo-piloto de
reconciliação terapêutica no momento da admissão hospitalar para, a partir dele, identificarmos os recursos necessários para a sua
implementação na prática clínica.
Material e Métodos: Estudo-piloto com 100 doentes admitidos num serviço de Medicina Interna entre outubro e dezembro de 2019,
com mais de 18 anos e a tomar cronicamente pelo menos um medicamento. A melhor história farmacoterapêutica possível foi obtida
sistematicamente, com posterior identificação, classificação e resolução das discrepâncias.
Resultados: A amostra em estudo, em geral polimedicada e com múltiplas morbilidades, apresentou uma média de idades de 77,04
± 13,74 anos, sendo 67,0% do sexo masculino. Foram identificadas 791 discrepâncias e as intencionais (95,7%) estavam documentadas
em 50,9% das situações. As dificuldades encontradas relacionaram-se principalmente com o acesso e a qualidade da informação
terapêutica e com a dificuldade de comunicação entre os diversos profissionais de saúde. Os principais recursos prioritários
identificados relacionaram-se com as categorias de processo, ferramentas e pessoal.
Conclusão: Os dados revelaram fragilidades nos registos clínicos disponíveis na interface dos cuidados primários/hospitalares. A otimização
das fontes de dados, normalização e informatização do processo, atuação multidisciplinar e definição de grupos prioritários
foram identificadas como oportunidades de otimização.; Introduction: The purpose of medication reconciliation is to promote patient safety by reducing medication errors and adverse events
due to medication discrepancies in transition of care. The aim of this pilot study of medication reconciliation at the time of hospital
admission was to identify the necessary resources for its implementation in clinical practice.
Material and Methods: Pilot study with 100 patients admitted to an Internal Medicine department between October and December
2019, aged 18 and over, and chronically taking at least one medicine. The best possible medication history was obtained systematically,
with subsequent identification, classification and resolution of the discrepancies.
Results: The study sample, in general characterized by polypharmacy and by having multiple long-term conditions, presented a mean
age of 77.04 ± 13.74 years, being 67.0% male. Overall, 791 discrepancies were identified. Intentional discrepancies were 95.7% and
50.9% of them were documented. The difficulties encountered were mainly related with the access and quality of therapeutic information
and communication problems between different healthcare professionals. The key priority resources that were identified were
related with the process, tools, and personnel categories.
Conclusion: The data revealed weaknesses in the clinical records available at the primary/hospital care interface. Optimization of
data sources, standardization and informatization of the process, multidisciplinary approach and definition of priority groups were

Title: Potential herb-drug interactions found in a community pharmacy patients

Abstract: Phytotherapy has always played a leading role in therapeutics. However, a strong knowledge of the risk-benefit
relationship of herbal products by patients and health professionals is necessary.
The goals of this study were to characterize the consumption pattern of medicinal plants in patients in a community
pharmacy, identify potential herb-drug interactions, and establish a list of recommendations for health professionals
and/or patients in order to prevent/minimize negative outcomes arising from these interactions.
In a sample of 25 patients, 24 cases of potential herb-drug interactions were detected. Most of the cases corresponded
to the simultaneous use of a conventional medication with Ginkgo. The recommendation list involved monitoring
the effectiveness and safety of conventional therapy associated with the use of medicinal plants, with particular
focus on clinical and laboratory monitoring of the occurrence of bleeding, blood pressure, glycaemia and liver
function.
The results indicate that there is still a lack of knowledge about the possible risks of herbal products, which may
adversely affect patient’s health. The pharmacist is in prime position to raise awareness in the population, avoiding
harmful situations for the patient's health.; A fitoterapia desempenha um papel preponderante ao nível da terapêutica. Mas só um forte conhecimento da
relação benefício-risco dos produtos à base de plantas, por parte dos doentes e profissionais de saúde, permite
retirar um máximo benefício desta terapêutica.
Foram objetivos deste trabalho a caracterização do padrão de consumo de plantas medicinais em utentes de uma
farmácia comunitária, a identificação de potenciais interações planta-medicamento e a criação de uma lista de
recomendações para o profissional de saúde e/ou doente, a fim de reduzir/evitar resultados clínicos negativos
decorrentes da ocorrência dessas interações.
Numa amostra de 25 utentes foram detetados 24 casos de potenciais interações planta-medicamento. A maioria dos
casos correspondeu à toma concomitante de um medicamento convencional com Ginkgo. A lista de recomendações
envolveu a monitorização da efetividade e segurança da terapêutica convencional associada ao uso de plantas
medicinais, com destaque para a monitorização clínica e laboratorial da ocorrência de hemorragias, da pressão
arterial, glicémia e função hepática.
Os resultados obtidos indicam que há ainda uma falta de conhecimento quanto aos riscos dos produtos à base
de plantas, que pode influenciar negativamente a saúde de qualquer doente. O farmacêutico está em situação
privilegiada para a sensibilização da população, evitando situações prejudiciais para a saúde do doente.2015-01-01T00:00:00Z
